After Prom Committee of 2012-2013 Scholarship
Guidelines:
This scholarship is available for all graduating seniors from Fertile-Beltrami High School. Two $500.00
scholarships will be awarded to seniors pursuing a degree at an accredited 4-year college or university.
Below is additional information about the After Prom Committee of 2012-2013 Scholarship:
Application: The application guidelines and cover sheet are available in the Counselor’s office or the F-B
Website.
Application Deadline: ALL APPLICATION MATERIALS MUST BE RECEIVED IN THE
COUNSELOR’S OFFICE BY 4 P.M. ON APRIL 1ST.
Selection: The Fertile-Beltrami Education Foundation Scholarship Committee will select the recipient.
Criteria:
The application must:
1. Plan to further their education at an accredited 4-year college or university
2. Demonstrate high character, leadership skills, motivation, initiative and respect
for others
3. Have a cumulative grade point average of 3.0
4. Be accepted at an accredited 4-year college or university
Award: Two $500.00 scholarships will be awarded to graduating seniors. The recipients of the After
Prom Committee of 2012-2013 Scholarship will be announced at the Honors Banquet held every year. A
check for $500.00 will be available in the F-B School office to be picked up by the recipient following
completion of the first semester of the student’s first year with proof of college attendance and passing
grades.

Application Cover Sheet
Application for the After Prom Committee of 2012-2013 Scholarship consists of five parts. Please submit
the following information and return to the Counselor’s office by 4 p.m. on April 1st.
1. Completed application cover sheet.
2. A copy of your high school transcript, signed by a school official.
3. A resume of your high school, community, and religious activities, include any offices held in
the organizations you have belonged to while in high school. Please include activities that
demonstrate your leadership skills, initiative, and respect and caring for others. This resume
can be more than one page long.
4. One letter of recommendation from an adult who is not a relative, but knows you, perhaps has
worked with you, or knows your involvement in activities and/or organizations.
