2013-2014 ENROLLMENT FORM AND AGREEMENT
I hereby apply for enrollment of my minor child________________________as a_____ grade student
at Wilshire Private School for the school year from September 4, 2013 to June 13, 2014.
The terms and conditions of said enrollment are set forth within this 2013-2014 Enrollment Form
and Agreement (“the Agreement”) as follows:
1. I agree to pay to Wilshire Private School all non-refundable applicable tuition, fees, and charges,
including:
(a) Application Fee of $150.00. This fee will be charged to all new students for the first school
year and must accompany this form. (Additionally, a $100.00 Assessment Fee for new
students is required.)
(b) Annual Registration Fee $200.00. But for the returning students who register by March 28,
2013, an early bird rate of $150 will be applied.
(c) Annual book fee $275.00 is a fee to cover textbooks, workbooks, and related materials, due by
August 1, 2013.
(d) Tuition for the 2013-2014 school year is as follows:
Jr. Kindergarten & Kindergarten - $ 6,500.00
1st through 6th grade - $7,500.00
(e) Extended Day Care Hours: $ 50.00 per week (3:45-6:30 P.M.)
(f) A tuition discount of 10% will be provided for the second child in your family who is enrolled
at Wilshire School and 20% for each additional child.
(g) Annual PTO dues of $55 and the Student Activity Fee of $120.00.
2. I understand that I may choose one of three tuition plans. (Please check one plan.)

of each month, September – May
* Annual Book Fee, PTO dues, and Student Activity Fee and the 1st month’s tuition (for September 2013) with
applying processing fee are due on August 1, 2013.
** The monthly and semester payment plan is for the convenience of parents. In the event of the early
withdrawal of a student, parents are responsible for the entire balance of the year’s tuition.

3. I further agree and understand that there is a 5-working-day grace period for each payment plan. If
tuition is received by Wilshire Private School after the 5th working day of the month, there will be a
$30.00 charge for late payment. I further understand and agree that there will be a $30.00 charge for
each returned check. I understand and agree that any payment not received within 15 days of the due
date may result in the voiding of the contract by Wilshire Private School. I also understand students
will not be allowed to attend classes when tuition is 30 days overdue. A student’s tuition payments
must be current in order for her/him to be permitted to receive semester grade reports.
4. I further understand, acknowledge and agree that Wilshire Private School is a school of limited
enrollment. By executing this Agreement, Wilshire Private School agrees to provide a place for my
child in his or her respective class, which may deprive another child of the privilege of enrolling at
Wilshire Private School. Further, in the event that my child withdraws from Wilshire Private School,
Wilshire Private School may be unable to enroll another child to fill my child’s place in the classroom.
Hence, I agree that all obligations shall be continued, and this Agreement shall not be terminated for
any reason other than an absence or withdrawal of my child due to a catastrophic injury or illness to
the minor child or to the undersigned.
5. Wilshire Private School reserves the right, at its sole discretion, to suspend or dismiss the abovenamed student if the student’s presence at Wilshire Private School would be detrimental to the student
or the school. In such event, parents are responsible for the remainder of the school year’s tuition and
fees.
6. My signature on this Agreement indicates my understanding of my responsibility and my
commitment to pay for a full year’s tuition.
