HONORS ENRICHMENT CERTIFICATE FORM, 2015-16

The Honors Enrichment Certificate is intended as an incentive to encourage Honors Program students to take advantage of opportunities for educational, cultural, social, and service enrichment activities and thus to make the most of their education while at IVC. Only events officially sponsored or endorsed by the Honors Program qualify towards credits, but students may propose upcoming events for consideration. Certificates will be presented at the Honors Program Awards Ceremony in May.

Enrichment Certificate completion may be a factor in choosing recipients of Honors Program

Scholarships and in assigning leadership positions.

In order to earn an Honors Enrichment Certificate, students must do the following:

Earn a minimum of 100 points by attending a minimum of 5 officially endorsed Honors

Program Enrichment Events or activities during the 2015-16 year.

Events will include cultural, educational, service, and social events, and allotted points will range from 10 to 25 points per event. The Honors Program will attempt to publicize the number of credit points before each event.

Attend the entire length of the event unless granted special permission beforehand. Point allotment may depend upon length of attendance.

Fill out an Honors Enrichment Activity Form for each event.

Attach materials showing proof of attendance and/or sign in at event as relevant .

Submit the Honors Enrichment Activity Form for an individual event to the Honors

Program Office within two weeks of the activity’s conclusion.

Forms may be in electronic or hard copy form although electronic is preferred. Electronic forms should be sent to honorsprogram@ivc.edu

, and hard copies should be submitted to the Honors Program Office in

Room A209.

The Honors Program will keep these forms on file, but students are also encouraged to keep a copy for their records and to keep track of their number of events attended. Submission of this form implies that you have actually attended the event described for the duration. Fraudulent representation may affect your Honors Program membership status.

Students may also propose events for official recognition as an Honors Program Enrichment Event. To propose an event, please submit an Honors Enrichment Event Proposal form to honorsprogram@ivc.edu

at least two weeks prior to the event when possible. Forms are available on the Honors Program website under “Honors Events and News” at http://academics.ivc.edu/honors/Pages/announcements-events.aspx

II. Event Evaluation

A. Attach and explain below proof of attendance if relevant (ticket stub, program, etc.). Not required when attendance has been marked on a sign-in sheet at the event.

B. Please briefly summarize what happened at this event and explain its value for you (100-150 words).

Some questions you might consider are: What did you get out of attending this event? What did you learn? What aspects of the event were most educational, enjoyable, useful, or interesting for you? Did this event tie in to any of your coursework (in Honors or regular classes), and if so, how? If this was a performance, did you think that the performance was successful? Why, or why not?
