12/26/14 LHJ: Calaveras County Deliverable Form E – 5-Year Action Plans Perinatal Mood and Anxiety Disorders (PMAD) (For further instructions see separate document titled “Action Plan Guidelines”) MCAH SOW Goal 2: Problem Category Perinatal mood/ anxiety disorders Problem Statement(s) Pregnant women and mothers of children under 2 years of age and their children are suffering from the effects of perinatal mood/anxiety disorder (PMAD) due to lack of awareness of PMAD, difficulty accessing mental health services, and providers' lack of awareness of the problem and available resources. Five Year Local Goal(s) Increased number of health care providers who routinely screen pregnant women and mothers of children under 2 years of age for PMAD and refer as needed. Increased number of referred women accessing resources as recommended by their health care provider. Risk/Contributing Factors Lack of education on the scope of the PMAD problem, lack of education on the signs/symptoms and risk factors, lack of education on the risks to the woman and to her family Best Practice Strategies/ Interventions All pregnant women should be asked about family and personal history of postpartum psychosis, severe depressive disorder and other psychotic disorders on entry in to PNC. Those at high risk should be assessed for s/sx minimally at 4-6kws and 3-4 months postpartum. A woman at high risk should have a plan in place that is shared with all of her pregnancy care providers.
