H.R.S. form 6800605 immunization record signed by licensed physician 2. Birth Certificate 3. Emergency information card submitted by your child’s teacher 4. Signed tuition agreement Attendance Parents are encouraged to schedule doctor’s appointments before school, after school or during school break. Excessive absences or tardiness will result in a loss of academic achievement and success. Please do not send your child to school if they are ill. Please call the office to inform us that your child will be absent. If your child becomes ill the school will call to make arrangements for student pickup. Medication It is extremely helpful to administer medication before and after school hours. However, if a student must receive medication during school hours the office staff will administer it only if prescribed by your child’s physician. Parents/guardians must sign a Medication Release Form for The Lake Mary Academy and Music to administer medication to a child. All medication must remain in the office. The Lake Mary Academy and Music takes all elements into consideration when delegating class assignments. Tuition and Fees Tuition for all students is due the first of each month. Payments are expected by the 5th day of the month. Please make payments accordingly. Report Cards and Progress Reports Each student is graded on his or her performance in the classroom at the middle and end of the year. Parent conferences are scheduled three times during the school year to review their child’s report card. Parents and teachers may request additional conferences at any time to review a student’s performance. Volunteers Parents are encouraged to visit the school, donate time whenever possible and to share their talents and knowledge with the students. Each classroom teacher needs a “Classroom Liaison” to help with various activities throughout the year. Lunch There are two options for lunch at LMAM. Students may bring a lunch from home or order the monthly lunch program. The Lake Mary Academy and Music 407-321-4447 LMAM Page 7 2/10/2016 The Lake Mary Academy and Music offers a healthy, catered hot/cold lunch program available through a local vendor. Lunches will be ordered monthly in advance. Menus will be presented the week prior to the upcoming month and the order must be paid in advance directly to the vendor. All lunches from home should contain healthy foods. Please keep in mind that it is difficult for teachers to heat or refrigerate food. All LMAM children are required to display appropriate manners during mealtime. We ask that parents reinforce good manners at home. Dress Code Uniforms are required for all students of The Lake Mary Academy and Music. The Lake Mary Academy and Music feels this eliminates competition and encourages unity. Compliance with the uniform requirement is mandatory. Please label clothing especially blazers, sweaters and jackets. Discipline Code According to the Department of Children and Families we are required to provide a plan of action for behavior that is consistent with The Lake Mary Academy and Music’s philosophy and foundation. Our discipline policy is to work with all children in a positive manner. We believe that all children need to have positive experiences that encourage academic achievement, as well as emotional and social development. Five Year Outlook The Lake Mary Academy and Music is continually working to direct its students to be successful members in our community. Our goal is to provide a stimulating environment that emphasizes and effective cultural enrichment in the form of music and art appreciation. The LMAM believes the importance of our accelerated curriculum and plans to expand our teachings to include Kindergarten. The Lake Mary Academy and Music asserts that at this very young age, quality education cannot be achieved in a crowded environment typically found at most public schools. As a result we are planning to add a Kindergarten within the next five years of service. Any families interested in joining our expansion efforts are encouraged to inquire.
