NAME:
All applicants must provide proof of Eligibility for the instructor certification process. This requirement
applies to new applicants and those inactive instructors returning to become recertified. Applicants should
turn in the completed documents in the order as listed below.

Check one option for the
safety program you would
like to be certified as an
instructor. Complete the
requirements as listed.
Proof of coursework must
be attached to the required
documents. See below.
Firearms/Hunter Education applicants: complete the web-based
Hunter Education safety online course Study Guide. Link can be found on
the process to become an instructor page – click on Firearms.
No fee is required to complete the Study Guide.
Bowhunter Education applicants: Complete the web-based Bowhunter
Education safety course. . Link can be found on the process to become an
instructor page – click on Bowhunter. *Print out the results of the Practice
Test prior to registering for instructor training session. No fees required to
be paid for the online course.
ATV/Snowmobile applicants: complete the web-based ATV or
Snowmobile safety online course Study Guide. Link can be found on the
process to become an instructor page – click on ATV or Snowmobile.
No fee is required to complete the Study Guide.

BCA background check to be completed. (Double sided)
Applicant fills out BCA POR form. (1 page)
Applicant reads and provides signature on adhering to
