DIABETES INFORMATION PRESCRIPTIONS – SYSTMONE
1. The trigger code for the Diabetes Information Prescription protocol on SystmOne is XaYbB (Diabetes selfmanagement plan agreed)
2. When this code is added to the patient’s record, the following question will appear. If any diabetes
information prescriptions are to be completed, this should be answered “Yes”.

3. The following template will then appear. For each diabetes information prescription, tick the relevant box
and click on the link to the appropriate letter template. NB. After each letter is generated, this template will
re-appear – when all relevant information prescriptions have been generated, click “Ok”.
4. The information prescription will then be generated for the patient:

5. The New Letter template will be pre-populated with the recipient and senders’ details. A letter type of
“Information Prescription” should be selected; the letter template will be pre-populated with the relevant
information prescription template selected in the Diabetes Information Prescriptions template.

6. The relevant letter template will then be displayed. The patient’s name and date of birth will be prepopulated, all other optional entry fields are marked with a grey box and can be typed in as appropriate.

7. Once the information prescription has been completed, this should be saved (using the relevant icon in the
top left-hand corner of the screen).
8. The relevant read code(s) for the diabetes information prescription(s) will be added to the patient’s record.

9. The information prescription(s) can be re-printed:
select the Communications & Letters node on the clinical tree  right-click on the appropriate letter (which
will have a letter type of Information Prescription)  select the Print option
