Rehabilitation Medicine Clinic Summary Points to Remember This clinic aims to help people with disabilities or reduced function to optimise their independence, social networks and quality of life. What Is the Rehabilitation Medicine Clinic? The Rehabilitation Medicine Clinic provides ongoing support to adults with various disabilities or difficulty managing day-to-day tasks because of an illness or medical condition. Many people are referred to this clinic when they go home from the RMH Inpatient Rehabilitation ward. What will happen at the clinic? Your first appointment and future reviews at the clinic will involve seeing a doctor with experience in rehabilitation. Follow up appointments will be arranged with you as required. It may be appropriate for you and your carer to attend appointments together. Please be aware that while every effort is made to keep to allocated appointment times, there may be unexpected delays on the day. There are a number of rehabilitation doctors that work in the clinic; you may not always see the same doctor each visit. The clinic may offer you various services including: Ongoing review of your health and ability to manage day-to-day tasks Reducing the risk of further complications or illness such as prevention of pressure areas or infection Education about your diagnosis, how your condition is likely to change in the future and your ongoing care Discussing your role in social groups and family and any difficulties in this area Assessing and providing advice about your thinking, memory and behaviour and considering medications that may help Reviewing your ability to drive and giving advice about driving assessments Helping you with returning to work Assessment of psychological aspects of disability Sexual counselling Referral to allied health professionals for gait aids, orthotics, home modifications and equipment or assistive technology Referral for community support groups or services and/or social benefits Referral to other specialist teams such as the RMH continence service and chronic wound service if appropriate Page 1 of 2 Who is eligible? We generally see people who live in the local council areas of Melbourne, Moreland and Hume but will see others outside of this area on discussion. How much does it cost? There is no charge for this service. How do I arrange an assessment? Referrals are generally made by the inpatient rehabilitation team when you go home from hospital. Referrals can be made via the Direct Access Unit (see details at bottom of page). How do I get there?
