MV Forms Description and Schedule 2015-16
Forms completed at the beginning of the school year:





MV Rights Notification Form
Student Residency Form to re-qualify all MV students (must include living situation)
Missing Documents Form: If needed.
Written Notification of MV Enrollment Denial: If student is turned down for MV qualification and
services.
Notification of MV Appeal Request: Shared verbally or in writing with parent/student when denied MV
qualification and services.
Forms completed at the end of the school year:



Unaccompanied Homeless Youth Verification letters: Provided to those students who are currently
enrolled in your district. Provided to seniors to give to their college’s Financial Aid Office.

Caregiver Authorization Form: When student is not living with parent or guardian. Liaison completes form to enroll
student.
Transportation Checklist: Go thru when setting up a new transportation situation. Keep for your own records.
District Cooperative Transportation Agreement: Set up with other districts when sharing the cost.
Best Interest Checklist: When liaison and caregiver/agency/school can’t agree on what school is the best fit for
students. Complete the best interest checklist together and come up with a solution.
MV Rights Notification Form: Twice while enrolled. Recommend sending at the beginning and end of the school
year. After twice commitment is fulfilled.
Written Notification of MV Enrollment denial: Completed by liaison whenever a student is turned down for MV
services.
Dispute Resolution Form: Given to student/family verbally or in writing to notify of their appeal rights when turned
down for MV services.
Missing Documents Form: Completed by student/ family. Send to prior school/agency, etc. to obtain missing
information.

Unaccompanied Homeless Youth Verification Form: Completed by liaison or counselor for students to give to their
Financial Aid Officers at colleges they are applying.
Student Residency Form: Sent to students to complete at the beginning of each school year.
MV Rights Notification Form: Sent to students/families twice while enrolled. Recommend sending in the fall and
spring of the first year as MV eligible. Make sure to document when and how notices were sent for your records.
Gas Voucher Receipt Form: Signed by parent/student any time a gas card is given. Form must be submitted with the
receipt for expense reimbursement.
MV Transportation Tracking Sheet: For liaisons to use for their own record keeping of transportation for each
student.
