Agreement to serve as Principal Investigator on Grants and Contracts Through the Colorado School of Public Health Purpose This agreement is to specify the obligations of faculty members of the Colorado School of Public Health (the “School”) who are eligible to serve as Principal Investigators (PIs) on grants and contracts through the School, based on the School’s policy for Principal Investigators, but who do not hold regular primary paid appointments in the School. Eligible faculty Eligible faculty who can apply for grants and contracts through the School are identified in Attachment 1. All such faculty must receive permission to submit a grant or contract through the School from the School’s Department Chair and Dean or designee in writing. Faculty with paid primary appointments (part or full-time) in the School do not need to sign this agreement. Faculty at partner institutions (CSU, UNC) will sign this agreement one time for any and all future grants and contracts. Other eligible faculty will sign this agreement on a case by case basis as needed, in advance of submitting a proposal through the School. Faculty with a primary appointment in another School or College of UC Denver and secondary appointments in the School and are eligible to submit grants and contracts through the School, but must obtain written permission in advance from the Chair of the Department in which the primary appointment is held. Faculty at partner or affiliated institutions who wish to submit a grant through the School must also obtain written permission from the Chair of their Department. An agreement regarding sharing of Facilities and Administration (F&A, also called ICR) revenue, if any, must be agreed upon prior to receiving the actual award under established School policies. 1 Approval – for Dean’s Office Use Only Approved by CSPH Executive Committee Richard F. If awarded, I understand that the award requires that I fulfill all applicable Federal, State, Grantor, and University laws, policies and rules pertaining to such grant or contract. In addition, I will comply with, and be eligible for, any applicable Department and School policies regarding distribution of Facilities and Administration revenue. I agree to comply with institutional policies and procedures relating to research compliance including obtaining appropriate IRB approval. Specific requirements may vary over time and by project. Current requirements are available in the School’s Dean’s Office. Should I fail to fully comply with the requirements stated above, I understand that the Department, School and/or University have the right to take any necessary steps to comply with applicable law, policy and rules. All such faculty must receive permission from the Department Chair and Dean or designee in writing. Such faculty must agree to abide by all applicable federal, state and University rules, regulations and procedures in order to be eligible. Persons in the Professional Research Assistant or Senior Professional Research Assistant ranks are not eligible to serve as PIs.
