Application must be typed. o The application file is set up in a table format to make typing/entry of information easier. Please be sure to read through all information/directions carefully prior to completing the application. All information must be completed and application signed by applicant attesting that information is applicant’s own work. OFFICIAL GRADES / TRANSCRIPTS A minimum nursing grade point average (NGPA) of 3.0 (calculated on prerequisite courses) is required after completing the fall semester (fall 2015) of the freshman year and must be maintained throughout completion of the program. All prerequisite courses must be successfully completed (grades of C or better, with the exception of CHEM 1000 with a B or better) by the end of Spring 2017 for the Fall 2017 nursing major component start term. Coursework completed prior to Fall 2015 The University of Wyoming must be in receipt of official transcripts from all other colleges/universities attended (including any dual enrollment while in high school) documenting all coursework completed prior to the fall 2015 semester. This includes transcripts for coursework that may have only included unsatisfactory grades and/or course withdrawals. In addition, official scores must also be on record with the university documenting any exam/test scores [e.g. Advanced Placement (AP), International Baccalaureate (IB), College Level Examination Program (CLEP), and Math placement (ACT, SAT, MPE). If not previously submitted, official transcripts and exam/test scores should be sent to the UW Admissions Office prior to the application deadline (December 11, 2015) and be accessible by the FWWSON by the application deadline (December 11, 2015). Should any new transfer coursework be completed, submission of new/updated official transcripts is required. Coursework completed during Fall 2015 The FWWSON obtains UW grades/transcripts as needed for all applicants; please do not request UW transcripts to be sent to FWWSON. Applicants who may be taking courses during fall 2015 through another school will need to have an official transcript sent to the UW Admissions Office documenting completion of that coursework. Transcripts must be received by UW and be accessible by the FWWSON by January 11, 2016. UNIVERSITY OF WYOMING ADMISSION Admission to the University of Wyoming (declared Pre-Nursing major – PNRS) is required as part of the application packet. Essays will be proctored by the FWWSON in early February on the University of Wyoming campus in Laramie. No exceptions to this requirement will be made. Applicants who meet specified Admission Requirements after fall 2015 will be contacted via UW email at the start of the spring 2016 semester in January regarding the essay scheduling process. Be sure to monitor email. Students will be given specific questions/topics to be addressed, provided at time of proctoring. BASIC BSN ASPIRE NURSING MAJOR ADMISSION FORM – DUE FEBRUARY 1, 2017 This form will be completed by Aspire students in the spring semester before they enter the junior year. Admission to the nursing major component of the program requires applicants to complete the Basic BSN Aspire Nursing Major Admission Form (nursing website, http://www.uwyo.edu/nursing, click on Programs, Basic BSN; Aspire; scroll down to this item listed under Admission to Nursing Major Component heading). o Due by February 1 of year in which beginning nursing major component of the program (fall 2017). o Form to be distributed by FWWSON Basic BSN staff prior to deadline. CNA CERTIFICATION – DUE FEBRUARY 1, 2017 All students entering the nursing major are required to have CNA certification in the spring semester, before they enter the junior year. Admission to the nursing major component of the program requires applicants to have active/current CNA certification (LPN Licensure can also satisfy this requirement) (nursing website, http://www.uwyo.edu/nursing, click on Programs, Basic BSN; Aspire; scroll down to this item listed under Admission to Nursing Major Component heading). o To be completed and accessible for verification by the FWWSON by February 1 of year in which beginning nursing major component of the program (fall 2017). o o o This is not an expectation to apply to the Aspire Program itself for spring 2016. Certification/licensure can be through any state in order to meet the application/admission requirement. o Please note that if certification/licensure is from another state, Wyoming certification/licensure is only needed if planning to work as such in Wyoming and to be legal to practice in the state. The School of Nursing will access certification/licensure records utilizing the online verification site through the State Board of Nursing identified on application to document compliance of this requirement by the application deadline. o Applicants may also want to verify that certification/licensure information is accessible online through the State Board of Nursing website to know that deadline has been met and that FWWSON will be able to locate and verify that information.
