Date: First Name (Print) Last Name (Print) Date of Birth Signature (or if a minor, signature of legal Guardian) New process FEI Documents – effective January 2016 – please submit as per below As equestrian sport develops and the number of events is constantly increasing, it is imperative to improve horse identification and traceability at events, during international travel and throughout the horse’s career. In order to improve horse document management in the FEI Database, we are developing new features which will clearly track all newly issued horse documents as well as follow up on all changes and modifications made to FEI Passports and FEI Recognition Cards. According to Art. 137 of the FEI General Regulations, National Federations are responsible for issuing FEI Passports and Recognition Cards, for assuring they are correctly completed and providing the identification pages to the FEI for registration; Art. 1001.11 and Annex II of the FEI Veterinary Regulations support this. Whenever a new FEI document is created, a change of ownership is made, a duplicate document is issued or any modification made which influences the FEI document, the relevant pages of the FEI document/national document will need to be uploaded to the FEI Database and these will all be saved to the horse’ profile. The pages that will be needed when certain actions are made are: FEI PASSPORT FEI RECOGNITION CARD FEI REGISTRATION NUMBER ONLY Page 1 of FEI Passport Page 1 of FEI Recognition Card Page from approved National (containing name) (containing name) Passport/other official document containing name of horse Page 10 of FEI Passport Page 2 of FEI Recognition Card Page from approved National (ownership details) (ownership details) Passport/other official document containing ownership details of horse Page 32-33 of FEI document Description and Diagram page(s) of Description and Diagram page(s) of (description and diagram of horse) approved National Passport approved National Passport/other 2016 FEI Horse and Rider REGISTRATION FORMS 2016 – FEI RIDER AND HORSE REGISTRATION official document Page 34-35 of FEI document Pedigree page of the approved National Pedigree page of the approved National (pedigree of horse) Passport Passport/other official document (if applicable) This will be put into place on the FEI Database in 2016; if the relevant document is not uploaded, the horse in question will be “pending” and will not be able to enter or compete at any FEI Event until the relevant document has been uploaded. Detailed guidelines will be provided to explain the process of uploading scans to the FEI Database as well as to explain in which specific situations it will be necessary. 2016 FEI Horse and Rider REGISTRATION FORMS
