DECLARATION
I understand that pursuant to the Data Protection Acts, 1988 and 2003 (the “DPA”), my consent is required for Horse Sport
Ireland to process personal data which is may have in it’s procession concerning me including disclosure to third parties such as
the Department of Agriculture Food and Marine. I note that this personal data may include sensitive personal data within the
meaning of the DPA, the processing of which requires my explicit consent. As such, I consent to the processing of any information
relating to me, either contained in this form or otherwise, for the purpose of issuing athlete licences and obtaining the necessary
DOCOM Certificates pursuant to the Tripartite Agreement.

*NOTE: FEI NAMING OF HORSES WITH PREFIXES/SUFFIXES
The FEI have confirmed their position regarding prefixes/suffixes as follows:
“The FEI does not consider a breeder’s or producer’s prefix or suffix to be a commercial name if these are included at birth in
the original stud book documents. However, including a breeder’s or producer’s prefix or suffix after the initial stud book
document was established without these prefixes/ suffixes is considered as commercial, due to the added value this could bring
commercially”.
The FEI fee for the addition of a breeder/producer prefix or suffix is CHF 1089 approximately €1088 dependent on current
exchange rate.
If applying for a FEI ID Number only, the above Prefix/Suffix fee does not apply, only the €30.00 registration fee applies. However,
once the FEI ID Number is upgraded to a FEI Recognition Card the FEI Prefix fee will apply if the Prefix/Suffix is added subsequent
to the initial studbook being issued. An FEI Recognition Card is required for all FEI competitions with the exception of horses
competing in their home country at CSI1*/2* (show jumping), CCI/CIC 1*/2* (eventing) and CEI1*/2* (endurance) and CPEID2*
(para equestrian). PARTICIPATION AT ANY FEI (INTERNATIONAL COMPETITION) IN A FOREIGN COUNTRY REQUIRES AN FEI
PASSPORT/RECOGNITION CARD (exception borrowed para-equestrian horses).
