ADDENDUM 1 DATE: November 20, 2012 PROJECT: HCPC FURNITURE INSTALLATION ITB NO: ITB 744-1309 OWNER: University of Texas Health Science Center Houston, Texas TO: Prospective Proposers This Addendum 1 is in addition to the previously posted bid documents and forms part of and modifies Proposal Documents dated, November 13, 2012, with amendments and additions noted below. 1. Section 2.2 PROJECT DESCRIPTION AND SCOPE is to be replaced as follows: 2.2 PROJECT DESCRIPTION AND SCOPE: The University of Texas Harris County Psychiatric Center is an acute care, in-patient psychiatric hospital. Prior to services being provided, the selected Contractor’s staff will be required to attend a free six hour hospital orientation, to be scheduled with hospital training staff. Work in the patient units can be conducted between the hours of 9:00 a.m. and 6:00 p.m. Contractor may however, be on site prior to and/or after the scheduled work hours to perform prep work. Contractor must maintain a minimum of four workers on the job site at all times. Free contractor parking may be available, however it is not guaranteed every day. All work is to begin as soon as possible and be completed by December 31, 2012. Contractor will be working in patient unit bedrooms. Patients will not be in the rooms while Contractor is working but will be in the Common Area of the unit. Provide the following: Each room has two beds secured to the concrete floor at the four corners with angle brackets and concrete anchors. Each bed is to be loosened from the floor and all anchor screws removed from the floor. All resulting holes must be finished flush to the floor. Paint the wall where the wood is attached with one coat of ICI Glidden Latex Paint. Install approximately 24 linear feet of 1x8 Southern Yellow Pine to one wall of each bedroom to secure the two beds and two desks. Both top and bottom edges of the 1 x8 wood must have routed edges; with quarter round shapes. Contractor will have the option of installing quarter round molding instead of routing the 1x8, but must specify this information with submitted bid proposals. ITB 744-1309 HCPC FURNITURE INSTALLATION PROSPECTIVE BIDDERS’ SUBMITTED QUESTIONS Secure the wood to the masonry walls using Pin Nails 1.5” long. Paint all wood with two coats of ICI Glidden Latex Paint. Install one “L” bracket to the footboard of the bed. Secure to the floor using concrete anchors. Secure the beds using Blue Concrete Screws 4” long. Secure the desks using Blue Concrete Screws 4” long. In order to bid this project, your company or one of your subcontractors must have had prior experience with projects of similar size and scope to the specifications outlined for this project. 2. QUESTIONS SUBMITTED AND RESPECTIVE RESPONSES: ITB 744-1309 HCPC FURNITURE INSTALLATION PROSPECTIVE BIDDERS’ SUBMITTED QUESTIONS o QUESTION 1: Per Item 1.12, no pre-bid meeting will be held; however, 1.12.1 states “A guided tour of the areas related to this project will be included as part of the conference agenda. This may be the only opportunity for potential respondents to view the Project site(s) before the submittal of Bids. Attendance at the pre-bid conference is not required.” Question: Please clarify. Additionally, is there an opportunity for a pre-bid walk-through to verify site conditions? ANSWER 1: All sub items of Item # 1.12 would not apply since there will be no Pre-Bid meeting held for this project. Opportunities for clarification were provided by email submission of questions prior to posted Questions Deadline. o QUESTION 2: The SOW states that “each bed is to be loosed from the floor and all anchor screws removed from the floor. All resulting holes must be finished flush to the floor. What is the finished surface of the floor? Is the contractor responsible for providing the tools to re-finish the floor? What is the quantity of rooms? ANSWER 2: The floor finish is VCT. If the concrete anchor does not come out cleanly and a small area of concrete is spalled off, a local patch may be required. This is the exception. Broken tile is also the exception. There are 120 rooms. ITB 744-1309 HCPC FURNITURE INSTALLATION PROSPECTIVE BIDDERS’ SUBMITTED QUESTIONS o QUESTION 3: The SOW states: Secure the wood to the masonry walls using pin nails 1.5” long. Question: Can contractor use different hardware instead of the pin nails? Pin nails won’t hold as well as screws and anchors. ANSWER 3: The winning contractor can propose alternate fasteners. Fasteners must not be accessible to the patients. Pricing should be based on the specified fasteners. o QUESTION 4: With respect to the painting, does the schedule permit painting prior to the other work in the rooms? ANSWER 4: All work in each patient room needs to be completed in one day. Please refer to item # 1 of Addendum 1 with reference to painting the wall in the patient rooms. o QUESTION 5: The SOW states that the beds and desks must be secured using Blue Concrete Screws. Question: Our experience proves that blue concrete screws do not use anchors. Do you wish for these items to be anchored to the floor or just screwed to the concrete? ANSWER 5: The blue concrete screws are used to attach the beds and desks to the wall. You will countersink the screw and screw through the bed, through the wood and into the concrete block wall. The same procedure is used for the desk but you do not need to countersink the screw into the desk. The winning contractor can propose alternate fasteners. Fasteners must not be accessible to the patients. Pricing should be based on the specified fasteners. o QUESTION 6: 3.5.1 states that “You must include with your bid a letter of intent from a surety company indicating your firm’s ability to obtain a payment bond for the entire construction cost of this project…The surety shall acknowledge that the firm may be bonded for each stage/phase of the project with a potential maximum of the entire construction cost. Bonding requirements are set forth in Article 5 of the Uniform General and Supplementary Conditions, see Exhibit B.” This SOW is not for the entire construction project, but rather a very small portion thereof. Will a payment bond in the amount of the work prescribed suffice? If not, what is the entire construction cost for this Project so that a payment bond may be obtained?
