GUIDELINES FOR NOTIFICATION OF A MAJOR MODIFICATION
OF THE CURRICULUM
Documents required for notification of a major modification of the curriculum
The following information should be provided as supporting documentation when notifying the
CACMS, when applicable, of a major modification to the curriculum:
1. A brief rationale for the planned modification to the curriculum. This would include the
explicit goals of the change (one-half page)
2. A brief description of the change (no more than one page)
3. A brief description of the methods used to evaluate the curricular change (one half page).
4. A brief assessment of the sufficiency of the following to accommodate the curricular
modification (one-half page):
a. Faculty effort and preparation
b. Resident effort and preparation
c. Student preparation
d. Administrative effort and preparation
5. A brief assessment of the sufficiency of the following to accommodate the curricular
modification (one-half page):
a. Educational space
b. Library resources
c. Student assessment
d. Information technology
e. Financial resources
6. A brief assessment of the sufficiency of the following to accommodate the curricular
modification (one-half page):
a. Clinical facilities
b. Patients (real and standardized)
c. Library resources
7. A brief assessment of the sufficiency of the following to accommodate the curricular
modification (one-half page):
a. Academic counseling services
b. Career counseling services
No appendices should be attached to the report. The complete submission should be
approximately four pages in length and no longer than six pages in length.
