Ain Dah Yung Case Management serves families with children who have severe emotional and behavioral disturbances. In a culturally sensitive Face To Face Health & Counseling manner, case managers help families: Develop a treatment plan and a crisis plan. Provide information about and referral to community Model Cities of Saint Paul, Inc. resources. Create a supportive team of family, professionals, and Metro Social Services, Inc. community members. Assist parents in advocating for their child’s mental health Thad Wilderson & Associates needs. Case Management / Initial Response A.H. Wilder Foundation “Initial Response” services offer assessment and service support in the first few months following a referral. Should longer services be needed the child is transitioned to another case manager for CLUES ongoing case management to help stabilize and improve the functioning of the child. Case Management: Technical Assistant Thad Wilderson & Associates This position assists Case Managers in: Completing and filing, in a timely manner, the required documentation for the out-of home and inter-state compact (ICPC) agreement Supporting and filing Court reports Meeting the requirements for Court Documentation in a timely manner Meeting the requirement for Title IV eligibility in a timely manner Appropriate use of the SSIS system Providing orientation and training for new Case Managers and new Supervisors, and Preparing for Case Managers consultation at which time funding requests take place. Children’s Crisis Response Children’s Crisis Response offers 24-hours a day, on-site response to children in homes, schools or institutions in Ramsey County. Services include: De-escalation and crisis intervention; a thorough mental health assessment to determine issues; and development of an initial crisis plan Follow-up: short term care up to two weeks following the crisis, including transition to ongoing treatment, and crisis recurrence reduction strategies Spanish and Hmong language supports Services provided by licensed mental health professionals or mental health practitioners under the supervision of a mental health professional Ramsey County partners with St. Paul Youth Service Bureau for Children’s Crisis Response Over 90% of home-based crisis calls have been stabilized with the child remaining at home Eligibility: All crisis services are available to anyone regardless of ability to pay or insurance type. A.H. Wilder Intensive therapeutic services are for families where a child’s problems interfere significantly with school performance and/or success in the community. The program is to focus on the Face To Face strengths and abilities of the family. Intensive therapeutic services are to present a non-threatening approach to the family, Metro Social Services lower stress and family conflict, reduce the chance of out-of-home placement whenever possible and appropriate, provide skills Thad Wilderson & Associates training such as anger control, parenting techniques, conflict resolution, stress management, communication, education of the family regarding the child’s mental health issues, and link the family to community services. Mentoring Plus and Other Youth Services entail individual mentoring, group mentoring, and/or support to the RCCMHC Bolder Options Family Partnership and Networking Program. Life Long Mentoring Services Metro Social Services, Inc. Ramsey County Children’s Mental Health Collaborative: 2015 Funded Services 2/13/2015 Page 3 of 5 Pages Ramsey County Children’s Mental Health Collaborative Funded Services for 2015 Local Collaborative Time Study (LCTS) Funds The Local Collaborative Time Study is a federal funding source intended to support the services provided by your local collaborative. It is a random moment time study administered by the MN Department of Human Services (DHS). It generates new federal dollars for Family Services and Children’s Mental Health Collaboratives. The LCTS accesses federal entitlement funds through two sections of the Social Security Act –Medical Assistance through Title XIX and Foster Care & Adoption Assistance through Title IV-E. Only Public Health, County or Community Corrections and School Districts who are members of an approved Family Services or Children’s Mental Health Collaborative can participate and earn federal dollars for a collaborative. Federal funds received from LCTS activities must be used to expand early intervention and prevention services for children and families. The goals of the services are: o Prevention of out-of-home placement o Enhancement of family support and children’s physical and mental health services o Development of a seamless system of services o Strengthening of local community–based collaborative efforts 2015 LCTS Programs 180 Degrees, Inc. Resiliency Mentoring Offering spring, summer and fall mentoring group sessions for a girls group and a boys group for the purpose of providing an opportunity for youth 1) to interact and bond with a peer group who have had similar experiences in a positive, mediated environment, and 2) to connect youth with positive community resources to assist them in gaining independence and identifying their strengths. Model Cities of Saint Paul, Inc. Families First Coaching Program To monitor, support, and link families in the Frogtown and SummitUniversity neighborhoods of Saint Paul to culturally specific community-based services, mental health services and activities that help them prevent crisis and learn how to successfully navigate challenging life situations that impact the child’s ability to learn. Coaching services will be tailored to meet the immediate needs of the family and are intended to reduce the impact that mental health issues have on children’s absences from school, while also increasing family stability. Northeast Youth & Family Services Northeast Therapeutic Services To help defray the costs of transportation services for NYFS day treatment program which is designed for youth ages 10-18 who need intensive mental health services delivered through separate middle school and high school programming. Youth receive three hours of therapeutic services daily and three hours of academic instruction at NYFS. Ramsey County Children’s Mental Health Collaborative: 2015 Funded Services 2/13/2015 Page 4 of 5 Pages Ramsey County Children’s Mental Health Collaborative Funded Services for 2015 2015 LCTS Programs Progressive Individual Resources Building Connections – Holistic Approach To expand the “Building Communities” program which provides a holistic approach to mental health difficulties of students of the African Immigrant and Refugee Students Charter School in Saint Paul. Services include: assessment of students identified as showing signs of PTSD, emotional and behavioral issues, peer relationship anxiety, and other generalized behavior problems; small group sessions for children; individual counseling; teacher training; and parental psycho-education information about their children. Su Familia Multicultural Counseling Prevention & Intervention of School Truancy To provide 12 Parent/Child groups a year (1x month) for Latino youths within Ramsey County who have been identified either through the schools or community corrections of being truant and needing improvement in school performance. Su Familia clinicians will also have available to them 100 hours to be used as either individual or family therapy with families needing additional support and mental health services. 2014 – 2016 LCTS Programs Canvas Health Services Therapeutic Assistance To attach an In-Home Skills Practitioner/Therapist to the Therapeutic Assistance Program/School based team in order to provide in-home skills and therapy to underinsured students and their families in the east metro area school districts, so as to improve outcomes, and maintain the gains made in the intensive school setting. This expands the mental health services provided in a partnership between Canvas Health and Intermediate School District 916 during the school day. Metro Social Services, Inc. Youth & Family Initiative To provide culturally-sensitive mental health education and support services to different immigrant groups through one-on-one assistance for the parents of youth with emotional and behavioral issues. It will also provide youth from immigrant families with culturally sensitive mentoring to help close any acculturalization gaps, acquire skills necessary to succeed academically, as well as socially while building healthy, positive relationships. Wilderson & Associates (2014-2015) River East Project To continue providing outreach and case management services to the River East students and families that are not accessing official targeted case management services from Ramsey County. These families are either not in county case management or prefer not to engage in county case management services. Through this families are offered the opportunity and time to develop trusting relationships with the school-based case manager.
