If you have an evening appointment, please ring the doorbell and await admittance. If you wish to cancel an evening appointment you must ring the surgery before 6.30 on the day of the appointment at the very latest. SERVICES AVAILABLE AT MITCHISON ROAD We offer a full general practice service including services for children and pregnant women, diabetes and asthma sufferers and patients with substance misuse problems. REGISTERING AS A PATIENT HOW TO REGISTER Please check with the receptionist to confirm you live in our area, and then complete the registration form in full with details of your previous GP and contact telephone numbers. If you are housebound, please let us know. 2 MITCHISON ROAD SURGERY A GUIDE TO OUR SERVICES TEMPORARY RESIDENTS If you need urgent medical attention whilst staying in our catchment area, you can complete a Temporary Residents form. Ask the receptionist for details. Disabled Access The majority of the building is fully accessible for disabled patients including a lift to the first floor and basement. Access for the Deaf and Hard of Hearing Patients A portable hearing loop is available for use by patients; please ask for this at Reception. Choosing a doctor Although you register with the practice, rather than with a specific doctor, you can nominate a doctor who you prefer to see – your “usual” doctor, and we will ensure that our records reflect your preference. You do not have to select your usual doctor when you first register, you can do it any time you like. If you have an on-going medical problem it is your “usual” doctor who will co-ordinate your care. MAKING APPOINTMENTS Appointments may be booked by telephone or by calling into the surgery. Appointments are 10 minutes long. You may consult with any of the doctors at the surgery, but we would encourage you to see your usual doctor for routine medical appointments. Please be on time for your appointments, if you are late, you may not be seen. Please also note that Reception may ask you the nature of your appointment to find out whether you would be better seen by a Nurse or other healthcare professional. Rapid Access Appointments As well as booking in advance we operate a system of “Rapid Access Appointments” enabling you to book a same day appointment. Please contact the receptionist if you need to be seen on the same day. Home Visits Please do not request a home visit if you are able to get to the surgery for an appointment. We try to reserve home visits for acute emergencies and housebound patients. Visits should be requested by 10:00am if possible. 3 MITCHISON ROAD SURGERY A GUIDE TO OUR SERVICES CANCELLING APPOINTMENTS PLEASE CANCEL YOUR APPOINTMENT IF YOU ARE UNABLE TO KEEP IT Please remember that if you repeatedly fail to keep your appointments we may have to ask you find a new GP. TEST RESULTS You can telephone the surgery between 11 am and 6.30pm during normal opening hours to obtain test results. REPEAT PRESCRIPTIONS Please give reception two working days’ notice for repeat prescriptions. Prescription requests should be made in writing, and delivered in person, or by post or fax. Please supply us with accurate details and a contact telephone number in case of query. Either tick the box from your last repeat prescription or use the special form available from the surgery. A three month supply is usually given. If necessary we can make temporary or permanent arrangements for a local pharmacist to collect your prescriptions and deliver your drugs for you. Please do not take up appointment time just to get a repeat prescription – plan ahead and order more supplies before they run out. From time to time the doctor will ask you to come in to be seen to review your medication. NURSING TEAM Please see the nursing team for: Blood pressure checks Asthma checks Cervical smears Childhood Immunisations Contraception and sexual health matters (including free condoms) and emergency contraception. HRT Checks Treatment of warts Healthy heart advice – including diet, alcohol and exercise and the ‘exercise on prescription’ scheme. New patient health checks Travel advice & injections – we keep vaccines in stock at the surgery, but please let us know your requirements a month in advance of travel. If you think you need more than one procedure, please tell the receptionist when booking. CONTRACEPTION AND SEXUAL HEALTH You can see the nurse or doctor for confidential advice on contraception and sexual health, including the “morning-after” pill. If you are under 16, please be assured that your consultation will still be confidential. ASK AT RECEPTION FOR OUR LEAFLET “TEENAGERS AND SEXUAL HEALTH”. BABY/CHILD HEALTH CLINIC These are held at the surgery on Wednesdays 1.00 to 3.00 pm. The clinic is for well babies and children under 5 only (unwell babies or children should be seen in any normal surgery). The clinic is run by a Health Visitor with a Practice GP and a Practice Nurse and are for; Advice Immunisation Developmental Checks Weighing Other benefits include; The opportunity to meet other mums and children of the same age Information on mother and toddler groups Information on local Childcare facilities We will send new mothers the date of their postnatal check appointment with the doctor, and the baby’s first appointment for immunisations with the practice nurse. If you would like to see the health visitor, please phone the surgery in advance to make an appointment. MINOR AILMENT SCHEME This scheme can help you if you require treatment for one of these illnesses Athletes foot Earache Indigestion Constipation Fever Insect bites & stings Cough Hayfever, allergic rhinitis Minor burns & allergies Cystitis Minor cuts Head lice Diarrhoea Mouth Ulcer If you would like to see someone without waiting for a doctor’s appointment, ask reception for a minor ailment voucher to take to your local community pharmacist who can help by supplying you with medicine from a list agreed with your doctor. If you are exempt from prescription charges, you will not have to pay for the medicine. If you have to pay prescription charges many of the medicines are cheaper to buy from the pharmacist. 5 MITCHISON ROAD SURGERY A GUIDE TO OUR SERVICES FORMS, MEDICAL REPORTS, EXAMINATIONS AND CERTIFICATES There may be a charge for these services. Please try to give us as much notice as possible when you need one of these services. FORMS – if you need the doctor to countersign forms for you e.g. Taxi Cards, or Freedom Passes, complete your part of the form and then post it to us or hand it to the receptionist. Please note we do not countersign Passport Applications. MEDICAL REPORTS – we will provide a medical report at the written request of an insurance company, prospective employer, or for your solicitor. Remember you cannot request these directly from us yourself. MEDICAL EXAMINATIONS – if you require a medical please ask at reception. We do not normally carry out insurance medicals – your insurance company should be able to advise you about these. CERTIFICATES – short term sickness does not need a certificate unless you have been off for more than 6 days. Your employer should have self-certification forms for you to complete. Long term sickness certificates do need an appointment to be renewed. Please try and book your appointment in advance of the date your existing certificate runs out. ESSEX ROAD ADVICE PROJECT They offer free, confidential legal advice on housing, welfare benefits, debt, employment and immigration. You can contact them at the Mary Ward Legal Centre on 020 7831 7079. CONFIDENTIALITY We respect your right to privacy and keep all your health information confidential and secure. You have a right to know what information we hold about you. If you would like to see your records, please ask at reception or call our practice manager. Remember you must ask us yourself, we cannot normally provide information about you to relatives. The practice abides by the principles of the NHS Care Record Guarantee further details can be found on our website. CHAPERONE All patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. This chaperone may be a family member, carer or friend. If you wish us to provide a chaperone it may be necessary to reschedule your appointment to a time when an appropriate member of staff is available. PRACTICE STAFF We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients. 7 MITCHISON ROAD SURGERY A GUIDE TO OUR SERVICES THE PRACTICE AREA This practice is within the Islington CCG area The area from which we accept new patients lies in the London Borough of Islington, as shown on the map above. We cannot accept new registrations from outside this area except in special circumstances. How to find us: The surgery is just off Essex Road, near the junction with St. Paul’s Road and Balls Pond Road.
