Oswego High School Prom Guest Pass Form
This form must be completed if you are bringing a friend to prom who is not an OHS student.
Guest Passes must be submitted to the dean’s office by the end of the school day Tuesday, April 7, 2015.
***Tickets will only be sold March 17th, 18th and 19th. Please do not wait to buy your tickets.***
Oswego High School Prom will be held on Saturday, April 11, 2015 at Jennifer’s Garden in Morris, IL.
Doors open at 6:00 pm. All guests must arrive before 7:00 pm. No one will be admitted after 7:00 pm.
Guests for prom must be at least enrolled in the 9 th grade and not over the age of 20. A valid high school ID must be
presented the night of prom (if guest is not enrolled in school, a valid state issued ID card must be presented).
Responsibility Statement
As an Oswego High School Student, I understand that all school rules apply at school dances and I take the
responsibility of informing my guest of these rules and the rules set forth in the prom agreement.
