NORTH HAVEN HIGH SCHOOL PROM 2014 Guest Information Form Dear Parent/Guardian and School Administrator, North Haven High School’s Prom is scheduled for Friday, May 9th from 7:00 p.m. to 11:00 p.m. at the Oakdale Theater in Wallingford. This is a positive event, and we welcome your child/student as our guest with your acknowledgement of our standards of conduct and attire. Please be aware that all guests must bring a photo ID, behave appropriately, and dance respectfully (if they choose to dance) throughout the evening. Formal attire is required and all students must arrive no later than 8:00 and leave/be picked-up by 11:00 p.m. when adult supervision will end. Guests must have the information form below completed and returned to the North Haven High School host student’s administrator before tickets can be purchased. The deadline for submission of these items is Wednesday, April 30, 2014. Parents, please complete the necessary information and sign below, indicating that you give permission for your child to attend the dance School administrators, please sign below indicating that the student honors your school rules and is permitted to attend your school dances.
