 All players must be current APTA members or pay a $20
Limited Membership Fee. To purchase a membership, call
the APTA Office at 888-744-9490 or apply online at
www.platformtennis.org.
 Draw size is limited. You will be included only with receipt of
your check. All Entries by 1/29/15.
 Favors, Lunch, balls, beer, prizes included
 Every effort will be made to ensure 3 matches per team
