JOB PURPOSE The aim of the Dundee CHP COPD programme is to improve the quality of life of patients with COPD. The post holder will ensure that the highest standard of care is given which complies with current legislation, clinical and local guidelines. Responsible and accountable for the holistic assessment, planning, implementation and evaluation of the treatment requirements of the COPD practice population using a variety of sources to meet the patients needs. The post holder will provide COPD specialist advice to healthcare professionals, clients, carers, relatives and other agencies within Tayside. 3. SCOPE AND RANGE COPD Peripatetic Nurses Dundee CHP has a population of approx. 160,000 patients across 29 General Practices. The prevalence of COPD within this population may be as high as 5%. 1. The post holder will be required to work autonomously with individual patients and their families, within the community setting, to co-ordinate and deliver services, to clients of designated General Practices in the clinic and home setting. 2. The post holder will have a Dundee wide remit. The area covered will include Dundee, Angus and Perth communities within the boundaries of the Dundee GP practices assigned. 3. The post holder is an autonomous practitioner, responsible for ensuring systems are in place to identify clients who are at high risk of developing COPD. She/he will also ensure that systems are in place to screen, monitor and diagnose and treat those at risk. General Conditions 05.08 4. The post holder will have a dedicated, experienced mentor from the COPD peripatetic service to support, guide and develop their role. 5. The post holder will act as a resource in relation to COPD for staff within Dundee CHP, mainly staff within designated practices e.g. GP’s, practice managers, practice nurses, community staff and other agencies e.g. social work department and voluntary agencies. 6. The post holder will be expected to participate in the ongoing development of the COPD service by initiating/participating in the development of the service through agreed audit and research. 7. Responsible for ordering essential supplies and equipment required to carry out the COPD programme such as equipment for diagnosing and monitoring disease e.g. spirometry consumables. 8. Responsible for ensuring the accuracy and maintenance of equipment used by the post holder e.g. spirometer and pulse oximeter. 5. MAIN DUTIES AND RESPONSIBILITIES 1. Responsible and accountable for the on-going holistic assessment, diagnosis, delivery, implementation and monitoring of COPD care, ensuring that the highest standard of service and care is delivered in a consistent and safe manner within the home / clinic setting. The aim being to manage symptoms, prevent deterioration in health where possible, prevent complications, improve quality of life and reduce hospital admissions. 2. The post holder will carry a housebound caseload incorporating all the practices assigned. This will be effectively managed and organised and workload will be prioritised to ensure that the best interests of the patients and service are met. 3. To support the public health agenda by promoting service development and participating in health education activities, promotion of healthy lifestyles and prevention of disease in order to achieve optimum health and independence for COPD patients, e.g. smoking cessation, pulmonary rehabilitation 4. Offer ongoing support to chronically sick patients and their families and provide support to those in crisis situations that may require immediate intervention. 5. The post holder will ensure that the care given complies with current legislation, national and local COPD guidelines. 6. Accurate, up to date clinical records will be maintained in accordance with divisional policy, GP practice requirements, legislation, good practice and patient confidentiality. 7. Develop, implement and evaluate systems that support and involve patients, carers and relatives in the planning and delivery of individualised care. Actively seek views of service users and carers to improve the health care experience for individuals. 8. The post holder is responsible to ensure a manageable workload is achieved, by ensuring patients are seen timeously and by the most appropriate person whilst achieving targets for the delivery of the COPD service. 9. Initiate, establish and maintain good working relationships and communications with patients and carers. 10. Engage in joint working with medical, nursing and PAMs staff to support a General Conditions 05.08 multidisciplinary team approach to care provision. 11. To work collaboratively with other agencies i.e. health, statutory and voluntary, to provide a seamless service to patients and carers being cognisant of local and national health agendas e.g. participate in the SSA process 12. Establish, maintain and review systems to ensure effective communication with individuals and within groups. This may be complex and potentially stressful, e.g. attendance at case conference to address complex needs. 13. To audit, against standards of care, measure clinical outcome and implement any required change. 14. The post holder is required to take responsibility for his/her own professional development in discussion with and the agreement of Respiratory Disease Facilitator. 15. By personal example, ensure that the highest standard of professional conduct is maintained. 16. Responsible for the ordering and maintenance of stock supplies e.g. respiratory consumables. 17. Responsible for ensuring that appropriate equipment is used, maintained, serviced and cleaned reducing the risk of cross infection, misdiagnosis and ensuring a safe working environment. e.g. calibration of spirometers. 18. Ensure effective use of financial resources and the delivery of COPD services. 19. To participate in and support ongoing research in COPD nursing practice audit / research. 20. To participate in the development, implementation and evaluation of policies, procedures and guidelines, in relation to COPD care at a local and regional level. 21. The post holder is required to comply with all statutory policies including Health and Safety of the division, board and the NMC code of professional conduct, guidelines and standards and to assist in the investigation and resolution of complaints in line with division policies. 6. COMMUNICATIONS AND RELATIONSHIPS The post holder requires to develop and maintain open and effective systems of communication on routine, complex, sensitive, challenging and potentially stressful matters on a daily basis. Using the full range of verbal, non-verbal and written communication skills, the postholder has a responsibility to enable any communication difficulties worked through and resolved in a positive manner. Reasons for Communication are varied and will include the following; Providing and obtaining information, educating, teaching, learning, listening, motivating, negotiating, persuading, empathising, breaking bad news, counselling and promoting change and development. Communication will at times be difficult as there will be barriers to understanding given the age and prevalence of co-morbidities within this group of patients e.g. patient understanding with regards to treatment regimes - daily. DECISIONS AND JUDGEMENT With the support of the dedicated mentor, the post holder will work as an autonomous practitioner, where the workload will be both self generated and generated via others e.g. GP's, district nursing service, hospital team, patient and carer demand, social work and other health care professionals. Assessing and interpreting acute and chronic patient conditions. Recommending prescription changes to GP's in order to improve patient management. Deciding when to refer to other health care professionals, statutory and voluntary services. Work will be carried out and reviewed in accordance with NHS Tayside policies and procedures. The post holder's manager will undertake performance review and appraisal in accordance with PDP policy. Meetings with manager are held on two - weekly basis. The post holder has the responsibility to regularly reflect upon and review their decisions and actions in relation to care management, communications and relationships. 11. The current salary scale for the post is £26,041 to £34,876 per annum (pro rata for part-time staff). Placing on the scale on appointment is normally at the minimum but may be higher subject to verification of previous relevant service. Salary is paid monthly by Bank Credit Transfer. The hours of the post are 28 per week. Start and finish times will be determined by the needs of the service. Membership of the NHS Superannuation Scheme is not compulsory but is open to all staff between the ages of 16 and 70 (65 in some instances). The contributions paid are a percentage of superannuable pay, which is essentially basic pay excluding, for example, overtime or travelling expenses.
