Why we decided to implement BOOST? BOOST is being implemented as part of Lodi Memorial’s Task Force developed to
prevent unnecessary readmissions.
Status of our Implementation Project:
BOOST team training has been completed. The BOOST Team has four task forces
functionning to address: 1) Target Assessment Tools 2) PASS Document 3) Follow Phone calls 4) Teachback. Boost trial will be implemented on a
selected Med Surg floor targeting AMI PNA CHF patients
Early success of our program:
Awareness. The program is causing a review and analysis of other organization programs, such as
Case Management, where improvements will be made.
