Foods Lab Make-up Form
Name: ___________________________________
Date: ______________________
Lab Missed at School: _________________________
Date of the Lab: _______________
To receive credit for a missed lab at school, the student must prepare the same lab at home.
The recipe must be attached and a parent/guardian and student need to complete this form. The
completed form and a small sample of the product must be included.
Just like any other class,
when a student is absent, the work or labs in Foods class, must be made up to earn credit. Please
see your teacher if you have any further questions or need assistance with different resources.
LATE WORK POLICY- If you are absent you are expected to get your homework the following
day, and have exactly the same number of days you were absent for you to complete the work.
Tests, quizzes, and labs need to be made up within one week of arrival back to class.
Bring back to class to earn credit:
1.
Completed lab evaluation with student and parent sections complete with parent signature
2. A small sample of the product (can be in a small disposable container or bag)
3. An attached copy of the recipe (including: title, measurements, directions, ingredients)
Student Evaluation: What made this recipe/lab successful? What problem occurred/what would
you change for next time?
Evaluate your work based on the following. Circle and assign yourself a 1 for poor, 2 for
satisfactory, and 3 for excellent in each category. Briefly explain your reasoning in each category.
Remember, you are here to learn, a rating of 1 will not necessarily lower your grade. It is ok to
make mistakes and to learn from them!
