


Coronary artery disease: history of stable or

Diabetic nephropathy with microalbuminuria > 30 μg./mL

Type 1 or 2 diabetes: initially diagnosed by fasting plasma
angina

multivessel coronary disease, history of

currently treated with insulin and/or other hypoglycemic
