Occupational Health, Safety and Wellness Department Employee Pre- Placement Screening Requirements Congratulations on your employment with the IWK Health Centre. The IWK Health Centre is committed to a safe and healthy workplace for all of its employees, physicians, students, volunteers and patients. Your pre-placement health screening is an important step toward ensuring your safety and must be completed before you start work. Please note that the IWK Health Centre has a scent free policy, please refrain from wearing scented products at all times while working. You will be provided with an Employee Confidential Occupational Health Record by Human Resources. This form is to be brought completed as much as possible to your appointment with the Occupational Health Nurse. It is your responsibility to obtain the following required information from your childhood/personal health records, Public Health, previous employers, your Family Physician or any health clinics that you may have attended. The required information is as follows: -MMR –Measles, Mumps, Rubella Documentation of two MMR vaccines Physician documentation of disease(s) Serology (bloodwork) indicating immune status -Varicella –Chickenpox Documentation of Varicella vaccine Documentation history of disease Serology (bloodwork) indicating immune status -Hepatitis B Vaccine (for direct care providers) Documentation of 3 doses of vaccine Serology (bloodwork) indicating immune status -Tetanus Diphtheria Pertussis Documentation of one dose within the last 10 years -Flu Vaccine Annual flu vaccination is strongly recommended -Tuberculosis (TB) Screening Documentation of a two step Mantoux skin test is required Employees who have received a BCG vaccination in the past are still required to have a two step skin test administered Documentation of chest x-rays results if done as a result of positive Mantoux testing You will be provided with a form for Mantoux (TB) Testing by Human Resources, please bring this form with you to your pre-placement health screening appointment The information will be reviewed at your pre-placement health assessment appointment. Tuberculin (TB) testing will be completed as needed by the Occupational Health Nurse at no cost to you. Please note that this will involve several visits for the testing and reading of the test. If you are missing any of the immunization requirements listed above, the Occupational Health Nurse will either offer these immunizations to you, or will advise you on the necessary requirements. Depending on the level of your occupational exposure risk, Hearing Testing or Fit Testing may also be required. This information is confidential and will be housed in your confidential health file within the Occupational Health, Safety and Wellness Department.
