Approval of the Minutes:
Minutes from July and August were approved as updated. September minutes were
approved with the following updates:
08-018: Tyler reported that the driver misjudged the turning radius of the vehicle.
The driver is no longer driving UW vehicles.
08-027: Leslie reported that the need for sidewalk repairs has been forwarded to
facilities but there is no timeline for repairs due to the number of repairs needed.
She had requested some sort of notice of hazard be posted but there has been no
reply as of yet.
U-Wide Meeting Recap:

Leslie brought up the subject of AEDs in the UW Administrative Policy
Statements. Jude informed the committee that the APS is under review for
updates. There is need for a formalized approval process via the AED committee.
The APS revisions should address the need.
Incident Reports:

2013-09-017: University Advancement. Ankle/equipment cart incident.
o Resolution: Satisfactory resolution.

2013-09-020: Sponsored Projects. Protruding instrument in pathway.
o Resolution: The area around the instrument will be taped off with reflective
tape to increase visibility and situational awareness. Satisfactory resolution.

2013-09-025: Education. Sidewalk trip on class excursion.
o Resolution: Employee will wear proper footwear for the environment.
Satisfactory resolution.

2013-09-029: Sponsored Projects. Foreign object embedded in eye while rotor
hammering.
o Resolution: Employee tried to self-manage but finally received appropriate
medical attention and is now working directly with supervisor. Committee
recommends safety goggles instead of safety glasses. Satisfactory resolution.

2013-09-054 - 055: Office of Minority Affairs. Student assistant’s vehicle rear-ended
by another vehicle in car pool lane. No injuries. Appropriate exchange of
information.
o Resolution: Satisfactory resolution.

2013-09-062: Law School. Tripping incident resulting in concussion. Medical
attention sought.
o Resolution: Kathy will follow up for further information.

2013-09-064: Athletics. Staff member who wasn’t supposed to be on field during
football practice received a football to the face which resulted in post-concussive
syndrome.
o Resolution: Security staff spoken with about allowing unauthorized and
untrained persons onto field during practice. Possible development of a
formal safety awareness training for persons to be present during practice.
Tyler will follow up for more information.

2013-09-068: Education. Carpel tunnel diagnosis possibly resulting from ergonomic
issues.
o Resolution: Work station ergonomic evaluation completed mid-September.
Satisfactory resolution.

2013-09-073: Sponsored Projects. Forklift/power line incident. Driver was welltrained but not enough spotters present. No injuries were sustained. It took 3 weeks
to fix the power line.
o Resolution: Methods to make breaker box more visible are being examined
and the group spoken with. Spotters will be added to the internal procedures.
Satisfactory resolution.

2013-09-079: UW IT. While installing a phone in Health Sciences an employee
exposed to woman in hospital gown and underwear who had some sort of fluid on
her.
o Resolution: Employee was not contaminated with fluids – just more
emotionally disturbed than anything else. Nothing to do. Satisfactory
resolution.

2013-09-086: Oceanography. Graduate student involved in spill of HC1 acid.
o Resolution: No physical harm and building evacuation and lab procedures
were followed properly. The shower was cluttered with items and that issue
has been addressed. Satisfactory resolution.
Good of the Order


Matt is working on the elections. Procedures are on auto-pilot and the process is
going well.
Leslie and Dian attended the Business and Academic Research Continuity
meeting. Look at Husky Ready on the Emergency Management website. It is
fully functional and ready for use. Scott will help with the process if you need
assistance.
