This is a tertiary government hospital with 300 bed capacity and is under the Ministry of
Health. As a staff nurse assigned in Nursery- NICU, I am handling newborn babies to an
average of 2-5 in 8-hour duty. Cases I have handled are babies born with PROM, Respiratory
distress, Meconium Stained Amniotic Fluid, Meconium Aspiration Syndrome, Hypoglycemia,
Hyperglycemia, High Bilirubin, Thrombocytopenia, etc.

STAFF NURSE (OB GYNE)/ INFECTION CONTROL COORDINATOR (OVERALL)

This is a private tertiary hospital with 400 bed capacity and is a JCI (Joint Commission
International) accredited hospital. As an Ob-Gyne staff nurse, I am handling an average of 14
patients with normal deliveries, Cesarian section, ERPOC (Evacuation of retained products of
Conception), Myomectomy, TAHBSO (Total Abdominal Hysterectomy with Bilateral
Salpingoophorectomy), PPH (Post partum Hemorrhage) Post Partum with Fever, Infections,
Breast Mastitis, and or DVT ( Deep Vein Thrombosis). Antenatal Cases like Pregnant with: UTI
( Urinary Tract Infections), Gestational Diabetes on Diet/ Insulin, Decreased Fetal Movement,
Uncontrolled Blood Pressure, Early Labor, Leaking / Premature Rupture of Membranes;
Pregnant for Induction of Labor.
RESPONSIBILITIES:

Assists the Level Charge Nurse in planning, directing, and coordinating the nursing
care for the patients in the designated area.

Assesses the overall needs of the individual patients, assists in the preparation and
implementation of nursing care plans, administration of drugs and medicines as
directed.

Maintains the continuity of care at the take over and hand over reports, ensuring
that the communication is clear at all times.

Ensures that doctors, orders and patient management plans are followed and that
the appropriate steps are taken, including immediate notification of the consultant
in charge or on- call, of any deterioration from the expected course of the patient's
condition.

Ensures that the appropriate records of the patient's progress are maintained
according to the unit's policies and procedures.

Prepares equipment and assist with examination and treatments, and clears away
the equipment as appropriate after use.

Escorts patients to other departments/ units as required.

Reports untoward incidents/ accidents / staff sickness to the Charge Nurse.

Ensures that the record of all drugs, IV Fluids, etc is up to date as required by the
Pharmacy, and ensures that the rules and regulations governing the storage and
custody of scheduled drugs and narcotics are strictly observed at all times.

Makes the necessary requisitions for replenishment of consumable stock items,
instrument repairs.

Participates in the discussion and implementation of clinical or administrative
problems arising in the unit, as part of the Quality Improvement Programme.

3. Sto. Nino Hospital-Daet Camarines Norte Philippines
This is a tertiary hospital which has an actual bed capacity of 100 and is Phil Health
Accredited. I have handled cases like Bronchial Asthma, COPD, Pulmonary Tuberculosis,
Hypertension, Myocardial Infarction, Diabetes Mellitus, Vehicular Accidents, CVA,
Appendicitis, Urinary Tract Infection, URTI.
RESPONSIBILITIES:

Gives direct nursing care

Plans, gives, and evaluates nursing needs of patient.

Carry out doctor's legal order.

Admits patients.

Performs such nursing activities such as bathing and oral hygiene.

Assists patients in moving, maintaining proper alignment of patient’s body
mechanics.


Observes signs and symptoms, institutes remedial action when appropriate and
record in chart.

Reports observation to the physician.

Observes and responds to patients emotional and spiritual needs.

Interprets to the patient and his family the role in promoting successful therapy and
rehabilitation.

Prepares patients for and assists physician when doing diagnostics and therapeutic
procedures.

Performs irrigations and catheterization and give enemas.

Administers and chart medication and patient’s reaction to medication.

Sees to it that the equipment receive proper care and maintenance


Performs post mortem care.

Receives and endorse equipment.

Maintains good help with hospital personnel.



