Youth Worker (Forensic health) located YHaRS Malmbsbury VIC.

Part Time position 0.5 EFT/ 38 hours per fortnight. Wed to Friday one week Thursday to Friday the second week (hours 9AM to 5PM). Fixed Term Contract until 28 February 2015

Assessment, Case Planning, Care Coordination.

The provision of assessment, case management, intervention, and ongoing support in order to facilitate sound health and wellbeing support to young people located within the Youth Justice Custodial setting.

KEY RESPONSIBILITIES

Direct Service Activities

The provision of a developmentally appropriate and responsive service to vulnerable young people within the Youth Justice System Custodial precinct;

Initiate and maintain contact with young people at risk of disengagement from services and community;

Implement a range of modalities including but not limited to: intensive individual casework, group work, family work, street outreach, collaboration and care planning;

Provide support to young people with substance use issues within a harm minimization framework;

Provide support to 12-15 young people using the case management framework;

Advocate on behalf of young people in the context of multidisciplinary health care planning and assessments;

Meet targets for completed episodes of care for;

Participate in the internal triage and assessment system;

Support the development and maintenance of systems to ensure service provision to family relationships;

Conduct comprehensive assessments;

Conduct evidence based interventions;

Facilitate groups for young people within the custodial precincts in relation to mental health and substance use problems specifically in relation to family disconnection, resilience development, emotional regulation, self care, problem solving, critical thinking and emotional regulation and their families;

Facilitate groups for young people around substance use and other issues;

Referral Networks and Pathways

Provide referral and links to a range of youth services including justice primary health, housing, legal, employment/educational and recreational services;

Develop and establish collaborative relationships with key stakeholders to enhance

Collaborative partnerships with relevant stakeholders are developed;

Provide secondary consultation to internal and external services;

Attend relevant meetings as requested;

Articulate YSAS relationships based approach with relevant stakeholders; formal/informal networks in order to achieve goals;

Administration

Ensure client files and database are up to date, accurate and meet both organizational and legislative requirements;

Ensure incident reports are timely and meet organizational procedures;

Complete administrative tasks, meeting organizational procedures as requested;

Participate in regular supervision with relevant Outreach Senior Worker;

Ensure OHS issues are recognised and acted upon;

Ensure all administration, data recording and financial accountability processes are known and adhered to, including administrative tasks related to COATS and other forensic systems;

Development of knowledge & skills through supervision, site meetings & professional development meetings;

Program Development and Evaluation

Key Responsibilities

Contribute to the development and maintenance of an innovative service delivery model for young people (from the target population);

Developing linkages with key stakeholders;

Where possible, include young people in the evaluation of the service;

Collaborate with other support services and community based activities to integrate support and provide optimal service provision;

Contribute to continuous quality improvement in relation to service delivery;

Ensure quality improvement is adhered to in line with YSAS procedures;

Ensure effective service is delivered and referral pathways are specific to the target group;

Compliance

Key Responsibilities

Work within the bounds of relevant legislation/regulations (e.g. Privacy, FairWork,

OHS, etc.) and relevant professional codes;

 Adhere to all YSAS’ policies and procedures including the Code of Conduct,

Confidentiality Agreement, EEO policy and Clinical practice guidelines;

Contribute to continuous quality improvement of the organisation.

Provide highest ethical standards at YSAS: not only to young people in the communities we serve, but to co-workers and ourselves;

Key Selection Criteria

1. Experience in working with complex and vulnerable young people experiencing problematic substance use;

2. Demonstrated understanding of the principals and practice of harm minimisation orientated treatment and prevention interventions, as well as knowledge of contemporary approaches to youth drug treatment;

3. Experience in working with the Youth Justice System and other Forensic Services;

4. Knowledge and understanding of adolescent development and ability to practice a range of interventions including; assessment, case work, care planning, advocacy, group work, crisis intervention and collaborative outreach based service delivery;

5. Highly developed engagement and communication skills with young people from a range of backgrounds and age groups;

6. Demonstrated capacity to work effectively both independently and within a multidisciplinary team setting;

7. Well-developed written, organisational and communication skills.

Qualifications and other requirements

Essential

Relevant qualifications Youth Work, Social Work, Family Therapy, Psychology or other health/welfare related qualifications; and / or extensive experience in the field;

Current Victorian Driver’s Licence;

Undergo a National Victorian Police Check;

Please note: Prospective employees are advised that they must have their Employee “Working With Children Check” as a condition of employment;

Desirable

Current First Aid Certificate (level 2).

I accept the PD as acknowledged above and understand that the PD will be reviewed by the year. I also understand that the PD may need to be amended occasionally due to variations in responsibilities and organisational requirements. Changes to the PD will be consistent with the purpose for which the position was established.

I agree to undergo a National Victorian Police Check prior to my appointment and hold a valid Working With Children Check throughout the term of my engagement.
