COMMON MISTAKES MADE ON SF86 – PLEASE READ PAGE 1: Date of Birth, Social Security Numbers and leaving the Middle Name Entry blank are the most commonly missed items. You must provide your FULL NAME, no Initials for Middle Name. If no middle name is available please list N/A. PAGE 2 - 3: Question 11, “WHERE YOUHAVE LIVED”: For any RESIDENCE entries in the last 3 years you must provide someone (Preferably someone who still lives in the area), DO NOT list a person related to you and do not use a reference more than once. PAGE 4: Question 12, “WHERE YOU WENT TO SCHOOL”: You must provide an entry even if your most recent degree or diploma was received more than 7 years ago (10 years for an SSBI), no matter when it was received. Also provide someone who knew you at the school (For schools you attended within the last 3 years) **For GUIDELINES on listing APO’s please follow this link: http://www.oconus.com/ZipCodes.asp or phone number Military Postal Service Agency (for APO/ FPO questions) call: 1-800-810-6098** PAGE 5 - 7: Questions 13 – 13A, “EMPLOYMENT/UNEMPLOYMENT”: For all your employment and residents please make sure that you are going back 7 years (If an SSBI go back 10 years) where requested, there are no date gaps, and for any unemployment or retirement please do not use yourself as the verifying source. You can use a friend, neighbor, spouse or relative but you must list someone and someone and do not leave “NAME OF EMPLOYER/VERIFIER” blank. For all your employment entries with the exception of UNEMPLOYMENT, please provide a “POSITION TITLE/MILITARY RANK”. Make sure that you also provide an “EMPLOYMENT CODE” and DO NOT use N/A for any entries. IF you are not sure of what code applies, use 9 (Other) and provide an explanation. If there are any employment gaps your form will be rejected. PAGE 9: Question 16, “People Who Know You Well”: Do not list your spouse, or any relatives, or anyone listed elsewhere on this form and do not list N/A or UNKNOWN. PAGE 10: Question 17, “MARITAL STATUS”: You must provide your spouse’s Social Security number and complete all fields required in this section. If spouse was born outside the U.S. indicate the type of documentation she/he possesses and the document number. Missing information will cause your form to be rejected. PAGE 11: Question 18, “RELATIVES”: If you are married please be sure to provide your in-laws even if they are deceased. Anyone listed born outside of the U.S. please indicate documentation type and the document number. This is also a common error that has been overlooked and will cause your paperwork to be rejected. PAGE 12 - 13: Questions 19 - 20C, “FOREING CONTACTS” – “FOREGIN ACTIVITIES” – “FOREIGN BUSINESS” - “FOREIGN COUNTRIES YOU HAVE VISITED”: This area is commonly skipped over and/or missed. Please complete all sections. PAGE 13: Question 21, “MENTAL AND EMOTIONAL HEALTH” – Please answer YES or NO to this and follow the directions in this section. PAGE 14: Questions 22 – 24, “POLICE RECORD”, “ILLEGAL USE OF DRUGS”, “USE OF ALCOHOL” - Please answer YES or NO to this and follow the directions in this section. PAGE 15 - 16: Question 26, “Financial Record” – Please follow the directions and DO NOT provide any information that is not requested, such as your Mortgage, Car or School loans. Only list the information requested pertaining to questions A through P that you have answered YES to. If you answer “YES” – You MUST provide all areas requested or your paperwork will be rejected and returned. Please use the “CONTINUATION SPACE” or an SF86A CONTINUATION PAGES to provide information you could not fit on the SF86. MEDICAL RELEASE: You MUST sign this form even if you answered “NO” to Question 21 “MENTAL AND EMOTIONAL HEALTH”. Any releases, areas requiring your signature, and Credit Report not signed will cause your SF86 to be returned to you for corrections. Also, be sure to type in your Social Security Number at the bottom of every page. Once you have completed the package you have a few methods on how to get it back to us. Even if you currently possess a Security Clearance, our customer still requires a new SF86 for every candidate submitted to our program.
