2012 BRAUNABILITY PREMIER CUP REGISTRATION FORM AND
TEAM ROSTER INSTRUCTIONS
NOTE: All registration pages are set up as forms. To fill in the forms, simply “tab” to the next field once you have
entered information. You are able to save these forms on your computer and fill them out at your convenience. To
submit/send the forms, save them as an attachment to an e-mail and send to register@tampathunder.org.
There are 3 pages that need to be filled out completely in this registration packet:
 Athlete/Coach Registration
 Family/Spectator Registration
 Totals Page
On the Athlete/Coach registration page, enter the team contact information for the registration process.
Then, enter the player name, jersey number, shirt size and banquet meal choice. In the “Fees” column, enter
the registration amount of $200.00 for each athlete, coach and assistant coach. This registration fee covers:



1 Tournament T-Shirt
3 Lunch Vouchers
1 Banquet Ticket
At the bottom of the Athlete/Coach registration page, sum the totals for fees for all athletes and coaches.
On the Family/Spectator registration page, enter the name, shirt size and banquet meal choice for each
spectator. In the “Fees” column, enter the registration amount of $100.00 for each family member/specator.
This registration fee covers:



1 Tournament T-Shirt
3 Lunch Vouchers
1 Banquet Ticket
NOTE: If a family member or spectator would prefer to only purchase a banquet ticket (no lunch
vouchers/tournament shirt), they may do so on this form by NOT entering a meal choice and shirt size. The
total in the “Fees” column for these family members/specators is $50.00.
At the bottom of the Family Members/Spectators registration page, sum the totals for fees for all family
members and spectators.
Finally, on the TOTALS page, enter the page totals from the Athlete/Coaches page and the Family
Members/Spectators page in the appropriate fields.
REGISTRATION DEADLINE IS MAY 15, 2012
Teams will have seven (7) days leniency on registration fees, but late fees will be incurred if fees are not
postmarked by May 15. Fees paid after May 15th will increase to $250.00 per athlete/coach, $150.00 per
spectator, and $75.00 per banquet ticket. This policy WILL be enforced at this event and NO exceptions will
be made.
