Service Name: Issue Disabled ID card

2 Service Description

Handicapped card is a personal granted by the Ministry of Social Affairs for people with a disability card, and is considered an official document indicative that the holder is disabled, in order to ensure his rights and services based on the law of persons with federal disability No. (29) for the year 2006, regulations and decisions issued in implementation thereof.

Conditions and Requirements to obtain service

Department of Welfare and rehabilitation of persons with disabilities.

The applicant will receive an email or SMS (if he has an account) and when the status will change

Submitted request is received by front desk officers who validate submitted data. If provided information does not fulfill requirements, request will be rejected.

Diagnosis Committee will verify the medical report.

5.

After successful data validation, Card will be issued to applicant.

8 The expected time to get service

Issued new card ten working days

Note:

The service may take longer time in case of shortage or lack of formal documents, medical and psychological reports

2.

If need more information document were not provided after 6 months from the date of the application, the request will be deleted. Except where required to provide additional information after reaching the age group or after the completion of the period of physiotherapy.

This service enables the person concerned to apply the issuance of card replacement of lost / corrupted for disabled and provided by the ministry in order to facilitate the extraction of cards procedures in the event of loss or damage.

Service Name: Issuing licenses for handicapped centers

2 Service Description

This service is to issue licenses disabled non-governmental centers that provide care and rehabilitation services for people with disabilities and early intervention services in addition to centers that offer individual sessions in Special Education and Rehabilitation Services language disorders, speech, and behavior modification occupational therapy

3 Point of Service Delivery

- Disabled Care and Rehabilitation Ministry Of Social Affairs

4 The Target Audience

Natural or legal persons (individual - for the company)

5 Documents Required

Fill your model applied for a license

• Copy of passport

• A copy of Family Book

• A copy of Qualification

• A copy of the trade license, if any

• Two (2) personal photo

A comprehensive study contains the following data:

1.

Educational curriculum that will be used in the enterprise.

2.

Methods and evaluation procedures and tests that will be used in the evaluation of cases

Detailed explanation of the therapeutic methods that will be applied in the organization.

4.

Full tuition fees and methods of payment and fiscal policy will be followed with regard to fees.

5.

The language that will be used in educational programs, curative and rehabilitative

6 Conditions and Requirements to obtain service

1.

The owner must be from nationals of the United Arab Emirates.

2.

May be the presence of non-citizens partners

3.

The center offers services authorized him to do what has been requested by a study of the project.

4.

Appointment of the administrative and technical staff according to rules and regulations in force in the ministry

5.

6.

Taking approvals for the appointment of staff

7.

That the building is appropriate for the type of service provided

As stated in the Council of Ministers Resolution No. (7) for the year 2010 in the system of nongovernmental institutions for the care and rehabilitation of the disabled (attached )

The Disabled department Employees processes the request and then checks the validity of the data provided, in case failure to provide full information, the application will be rejected.

4.

After the success of data validation, will conduct a survey of the site to ensure the quality of the facility.

This survey can be repeated in the case if there was a need to repair the damage.

5.

Upon completion of the survey of the site, will require the initial documents submitted and then the final.

Will be paid handicapped centers e-Dirham license fee

7.

The license will be issued to the applicant
