Additional documentation can be copied from : http://www3.shu.ac.uk/HWB/placements/Physiotherapy Placement team: Sue Green 0114 225 5575: Myles Butler 0114 225 2231: Jo Adams 0114 225 2490: Kathie Moore 0114 225 2404 © SHEFFIELD HALLAM UNIVERSITY 2011 ADDITIONAL COMMENTS: Telephone update: Monday 14/03/11 (Kath Moore and Kerry Anderson) Problem 1: - unfortunately not really improving - still needing a lot of prompting re assessment, problem solving and clinical reasoning Problem 2: - time keeping still an issue Recommendations: Problem 1: - continue to support development of clinical reasoning process by guided reading and questioning Problem 2: - keep a close eye on time keeping Review of objectives: Friday 18/03/11 Kath and Charli met first: Problem 1: - Charli felt things were improving and she was more focussed now on her placement since university course work was handed in on Wednesday - she is getting more used to being questioned and is doing the preparatory reading prior to practising a couple of assessments - she feels her problem identification and clinical reasoning is improving Problem 2: - there was one time keeping issue but she felt that was because of a misunderstanding / miscommunication issue Kath Moore, Kerry Anderson, Amy Bird and Charli Jackson: Problem 1: - there is a definite improvement. There has been a couple of assessments which have been supported with guided reading prior to the assessment and questioning following the subjective to allow for problem identification and clinical reasoning. These have been much better and questions have been answered appropriately. However, it was noted that they were very straight forward assessments. Problem 2: - there was an issue when Charli was a few minutes late when meeting up with another student to work on a case study presentation; the could be put down to a communication not being very clear. However, there have been other occasions when Charli is running down the drive to get in on the dot of 8.30am! Recommendations: Problem 1: - there will be more assessments for Charli to practice and the next patients will be more complex - Charli needs to show consistency and progress by successfully assessing these patients; she will be told patient details prior to the assessments so she can prepare Problem 2: - it was reinforced that she should be changed and ready for work at 8.30am and not just getting through the door at that time - continue to keep a close eye on time keeping and if there is another incident of lateness Charli will FAIL the placement Follow up: Kath to telephone again on Wednesday 23/03/11 to check on progress in the final week of placement Additional documentation can be copied from : http://www3.shu.ac.uk/HWB/placements/Physiotherapy Placement team: Sue Green 0114 225 5575: Myles Butler 0114 225 2231: Jo Adams 0114 225 2490: Kathie Moore 0114 225 2404 © SHEFFIELD HALLAM UNIVERSITY 2011 ADDITIONAL COMMENTS: Telephone update: Wednesday 23/03/11 (Kath Moore and Amy Bird) Problem 1: - unfortunately there have been no new patients for Charli to assess since the last review so progress with the objectives around this problem hasn't been possible Problem 2: - has been early every day since last review so is now meeting this objective Recommendations: Problem 1: - by Friday morning there should have been the opportunity to assess three new patients - Charli will be told about each new patient prior to her assessment so she has the chance to read up and prepare for each of the assessments - she needs to be successful in meeting the objectives around this problem in order to demonstrate consistency of her assessment, problem identification and clinical reasoning skills in order to pass this Level 6 placement Problem 2: - monitor time keeping until the end of the placement Note: Clinical educator to ring me on Friday 25th March lunchtime for update as to Charli's progress Telephone update: Friday 25/03/11 (Kath Moore and Amy Bird) Problem 1: - unfortunately Charli has failed to improve on the additional assessments she has had to do this week and in fact was deemed to be unsafe by assessing a patient with Parkisnson's disease with the patient in sitting but with her feet not on the floor. She also made a patient cry by assessing straight leg raise with a patient with severe sciatica. She has therefore not met the set objectives she needed to meet in order to pass the placement.
