Couchiching First Nation Sports & Leisure Subsidy Application
It is the intent of this administration to support parents and encourage our children to pursue their sports and leisure
related goals. Through nurturing and building upon their skills we hope to provide them with brighter futures and
unlimited possibilities.
CFN currently provides subsidies to a maximum of $ 350.00 in two sports and leisure related categories.
1) Sports and Leisure Registration
2) Sports and Leisure Travel
In order to be considered for these subsidies, the child must have their own CFN Band Registration Number, be under
18 years of age and attending school on a regular basis.
1) Sports & Leisure Registration Subsidy: the parent/guardian must provide the Recreation Leader with
proof of the youth’s registration in an activity falling under the Couchiching First Nation Sports &
Leisure Subsidy Policy. Proof must be in the form of payment invoice/receipt indicating the amount
paid/received by the organization, the date received, activity registering for, the organizations
name/business type, and the organizations contact information, unless the organization is direct billing the
Recreation Department and is submitting all the required information. In this case the organization will
have a form for parents to fill out.
2) Sports & Leisure Travel Subsidy: the parent/guardian must provide the Recreation Leader with proof that
the youth attended the event they are requesting subsidy for. Acceptable proof consist of:
 Travel expenses receipts, fuel, flight tickets
 Food and lodging costs for child and chaperone, if applicable
 Chaperone/coach/team contact information
 Signed letter from the event coach that the child attended the event
