Doctoral Degree Licensure Information
How to Apply for a North Carolina Licensure
It is your responsibility to apply for licensure and to make sure we have all the necessary documents
for your licensure paperwork.
1. Complete the following forms
 Form V – Verification by Institution
 Complete the top three lines ONLY
*Your license will be mailed to the address you provide on these forms.
2. Request transcripts
 Request an official UNCW transcript from the Office of the Registrar
 Follow the instructions to request your official transcript through SeaNet
(http://www.uncw.edu/reg/students-transcripts.htm)
 Request a transcript to be mailed to Dr. Carol McNulty (do not request an electronic
transcript)
 Step by step instructions to complete the transcript request:
 Are you requesting that this transcript be sent to yourself? NO
 Enter the name of the individual: Dr. Carol McNulty
 Do you know and want to use an email address to identify your intended
recipient? NO
 The transcript for your licensure paperwork should be mailed to:

 If you have graduated or have taken education courses at another institution (that were
not transferred to UNCW), you must send an official transcript to:

3. Praxis II Scores
 Official Praxis II scores must be on file in Dr. McNulty’s office (if required for your
licensure area)
 Additional information on Praxis II is available at
http://www.uncw.edu/ed/advising/praxis.html
4. Licensure Fee
 Submit check in the amount of $55.00 made payable to “NC Department of Public
Instruction” or a Credit Card Authorization Form (PDF)
 Your check will not be submitted to DPI until we receive degree posted transcripts.
Degrees are generally posted by the Registrar at the end of June, August and January.
5. Submit
 Submit the following documents
 Form V
 Check or Credit Card Authorization Form
 We cannot process your licensure paperwork until we receive all the necessary
documentation.
 It is your responsibility to make sure the documents are submitted to Dr. Carol McNulty.

*Please remember to allow 2-3 months after graduation date for processing and receipt of
your license.
