JOB DESCRIPTION MENTAL HEALTH PRACTIONER THE POSITION A Mental Health Practitioner works under the direction and supervision of a Mental Health Professional, providing home- and community-based mental health services to individuals with mental illness. This position requires a high degree of self-initiative and motivation. The schedule is flexible, as determined by client needs, and involves driving to client homes or various locations within the community where services are provided. The Mental Health Practitioner completes assessments and develops treatment plans with clients served based on needs identified by the client and the assessment. The Practitioner should possess good verbal communication and writing skills, assessment and problem-solving abilities, knowledge of the mental health service system, organization and record-keeping skills, and the ability to locate information and resources for a variety of client needs within the mental health, medical, housing and social services system. SPECIFIC RESPONSIBILITIES 1. Establish therapeutic relationships with assigned recipients and meet with each client as needed up to 6 hours per week. 2. Under the direction of the Mental Health Professional, conduct functional assessments and develop individual treatment plans for assigned recipients. 3. Under the direction of the Mental Health Professional, develop and maintain recipient files according to the requirements of Minnesota Statute 2546B.0623. 4. Implement services in the treatment plan for each recipient under the clinical supervision of a Mental Health Professional. The plan may include services that instruct, assist, and support recipients in areas such as: interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, medication education and monitoring, mental illness symptom management skills, household management skills, employment-related skills, and transition to community living services. 5. Document all services provided accurately, completely and within required deadlines. 6. Under the direction of the Mental Health Professional, review individual treatment plans for all recipients, and revise them as necessary. 7. Complete initial and ongoing training requirements defined for the position at required minimum standards. 8. Meet with the Mental Health Professional Clinical Supervisor at least twice monthly for training and treatment plan review. 9. Report problems, concerns, client grievances, and potential maltreatment of clients as defined in Minnesota Statute 626.5572 according to the requirements of Minnesota Statute 626.557. Qualifications To qualify as a Mental Health Practitioner (MHP), one must meet at least one of the following criteria, Obtained a bachelor’s degree in a behavioral science-related field from an accredited college or university AND: Have 2,000 hours of supervised experience in the delivery of clinical services in the treatment of mental illness or children with emotional disturbances; or Has completed 6,000 hours of supervised experience in the delivery of clinical services in the treatment of mental illness; or Is enrolled as a graduate student in one of the behavioral sciences or related fields formally assigned to the center for clinical training by an accredited college or university; or Obtained a master’s or other graduate degree in one of the behavioral sciences or related fields from an accredited college or university Salary and benefits Competitive salary dependent on experience Health and dental benefit package Short-term disability Life insurance Simple IRA with employer matching
