It prepares the student to function in the role of a Nurse Assistant under the supervision of a Registered Nurse or Licensed Practical Nurse. This course was designed to meet the curriculum requirements of the Michigan Department of Community Health. Satisfactory completion of the course makes the student eligible to take the State certification examination. Students must successfully complete 75 hours of theory, lab practice and clinical practicum. Student will also learn proper interviewing skills; resume writing, CPR and First Aid. CPR is included in the above stated tuition. Students are responsible for the cost of supplies (pen, notebook), and uniforms. Basic Life Support for Healthcare Providers Class (BLS) Tuition: $50.00 This class is designed for licensed or non-licensed health care professionals such as physicians, nurses, EMT, and First Responders. This class requires skills and written exam to receive BLS provider certification card. Class time ranges from 4 hours. This class will be included in the Nurse Aide Training Program. Work Skills Training : Tuition: no charge This class will give students basic information and understanding of Resume writing, online job applications, how to dress for interviews, and interviewing skills. Enrollment Dates for 2013-2014 Nursing Assistant Program: October 7-23, 2013 March 31-April 16, 2014 October 6-22, 2014 November 4-20, 2013 April 28-May 14, 2014 November 3-19, 2014 December 2-18, 2013 June 2-18, 2014 December 1-17, 2014 January 6-22, 2014 July 7-23, 2014 February 3-19, 2014 August 4-20, 2014 March 3-19, 2014 September 8-24, 2014 3 Volume 2 10/27/13 Enrollment: Classes are taught in units; therefore enrollment is continuous. Enrollment Requirements: Students should have a high school diploma or GED. Students under the age of 18 must have the signed permission of a parent or legal guardian. Each student must present a valid Michigan identification and must pass a criminal background check. Students are required to have TB skin testing with a negative reading and/or chest x ray if indicated. It is also strongly recommended for students to complete a Hepatitis B vaccination series before clinical rotation. Tuition/Fees: Total cost of the Nurse Aide Training Program is $600.00 which includes registration fee. A registration/ non-refundable processing fee of $300.00 are due at time of registration, which is included in the total tuition cost. Classes are on a first come, first serve basis, and the registration fee is required to reserve your seat. Full tuition is due no later than the Friday before clinical practicum. Payments must be made on Monday mornings before the start of class, if no payments made, students will not be allowed to attend class. Payments can be made by VISA, Master Card, Money Orders, Cashier’s Check. No cash will be accepted. For payment plans please visit school office. Student Fees: Students are responsible for purchasing, uniforms, supplies (notebook, pencils, pens, stopwatch), and State exam fees. *See course syllabus for information on purchasing books, uniforms and State testing fees.* Calendar: The school will be closed on legal holidays that may fall on a weekday: New Year’s Eve, New Year’s Day, Good Friday, Memorial Day, Fourth of July, Labor Day, Veterans Day, Thanksgiving and the following day, Christmas Eve and Christmas Day. School Hours: Monday-Friday 8am-4:00pm Day Classes (3 weeks) Monday-Friday 4pm-9pm Evening Classes (3 Weeks) Closed Weekends & Holidays Office Hours: Monday-Friday 10:00am-4:00pm by Appointment only 4 Volume 2 10/27/13 TB Testing: TB skin test is required before any student can begin any clinical rotation. Results must be negative and/or confirmed by a chest x ray. TB testing can be done at a primary care physician office or local Health Department. Students must bring documentation before the first day of class no later than the week before clinical rotation. This is required for clinical at long term care facilities. Hepatitis B Vaccine: It is strongly recommended for students to complete (or in the process of completing) the Hepatitis B vaccination series before clinical rotation. Hepatitis B vaccine can be obtained by primary care physician office or local Health Department. Students are responsible for the cost of all testing and vaccines. Criminal Background Check: Students who are accepted into the Nurse Aide program must pass a criminal background check. In some cases, clinical sites require this documentation in order to start the clinical practicum in approved long term care facilities. Any student with history of abuse or neglect towards patients/residents, or have certain felony convictions may not be able to obtain employment at long term care facilities. Students are responsible for the cost of criminal background check and/ or fingerprinting. The cost for Criminal Background is $10.00. Credit Allowed from Previous Education and Training: No credit will be allowed or accepted from previous education or training. Attendance Policy: Class lecture, lab and clinical attendance are Mandatory. Class attendance is from beginning to end of class, and full positive participation is expected. Students are allowed one excused absence during the course of class lecture. In order for the absence to be deemed excused, students must provide supported documentation such as a letter from physician verifying illness or other health matters. If there is more than one absence, the student must complete the program during the next class session if space is available. If unable to attend next session, tuition will not be refunded, and students will be responsible for full tuition and fees for the next class session attended. 5 Volume 2 10/27/13 Clinical Practicum Attendance: Clinical attendance is Mandatory. If a student is absent during clinical, that student will not receive a certificate of completion and must repeat the entire clinical practicum during the next class session, if space is available. During that time students have 45 days to complete the program with no additional fees. If student unable to complete Clinical Practicum during the 45 day period, students must complete the entire program. Students must readmit the program as a new student and is responsible for Full tuition. Tardiness: Students are considered late if he or she arrives 5 minutes after class starts. Students are allowed to be tardy twice, three or more times are considered an absence and students are responsible for make-up work and missed assignments. If testing, students are asked to remain outside the classroom until testing is over if he or she is tardy to class. Students should not be tardy to clinical, NO EXCEPTIONS! Inclement Weather: In an event of severe weather conditions, class may have to be cancelled. Students will be called and or emailed in such situations. Tardiness during severe weather conditions will be taken into consideration. Make-up Work policy: If students are absent from class/lecture, the student will be required to make up all missed assignments and or test the next day of class attendance. Any lab time that is missed students must make up this time for it is mandatory to complete a certain amount of lab hours to receive a certificate of completion. There is no make-up work for clinical practicum, students must complete clinical during another session within 45 days. Cancellation/Withdraw Policy: 6 Volume 2 10/27/13 Advanced care Training holds the right to cancel classes for any program due to low enrollment or any other circumstances. Students who are affected by cancellation will be given the option to attend another session. All tuition and fees paid by the applicant shall be refunded if requested within three business days after signing a contract with the school. The tuition and fees paid by the applicant shall be refunded if the applicant is rejected by the school for enrollment. An application fee of not more than $25.00 may be retained by the school if the application is denied. All refunds shall be returned within 30 days. Students who withdraw from the program within three business days of signed contract are subject to full refund except $25.00 application fee. *see refund policy for students who withdraw after three days of signed contract. Leave of Absence Policy: If a student must take a leave of absence during the course of the training program will be allowed to reenter during the next class sessions if space is available. There will be no additional fees. All outstanding balances must be paid prior to re-entry. Students have 45 calendar days to re-enter and complete the course, or all prior tuition and fees will be forfeited and students must reapply as a new student. Students must provide a written documentation for leave of absence in order to be considered for future sessions. Suspensions/Reinstatement policy: Students are not granted suspensions. If students cannot follow rules and regulations of the program, the student will be terminated from the program. Students can appeal the decision of termination under the guidelines of the appeal/grievance process. Students may apply for re- admission however, acceptance is not guaranteed. A student, who was terminated from any program and is approved for reinstatement, must re-enter the program within 45 days, with all balances paid. If students fail to reapply within the 45 day period, he or she will have to enroll as a new student and pay all tuition and fees associated with the program. Refund Policy: The tuition and fees paid by the applicant shall be refunded if the applicant is rejected by the school before enrollment. Examples of rejection may be but not limited to criminal history, or rejection from previous training programs. An application fee of not more than $25.00 may be retained by the school if the application is denied. All tuition and fees paid by the applicant shall be refunded if requested within three business days after signing a contract with the school. All tuition and fees paid by the applicant shall be refunded within 30 days. Once the three business days have elapsed after student signed contract, the following policy becomes effective: Three Day Cancellation: An applicant who provides written notice of cancellation within three business days (excludes Weekend & Holidays), of executing the enrollment agreement is entitled to a refund of all monies paid except $25.00 of application fee 7 Volume 2 10/27/13 After Three Day Cancellation: Students who withdraw after the third business day will not receive any refund; however students may apply funds to future sessions within 45 calendar days. If students fail to enroll in future sessions all monies will be forfeited by the school. Grading Systems and Reentrance conditions: Advanced Care Training uses a percentage grading system. Students must pass all exams with a 75% or higher and pass the lab and clinical practicum with 100% accuracy in order to successfully complete the Nurse Aide Training Program. Students will be evaluated for Lab and Clinical using the Student Achievement Record. Student assignments are evaluated weekly. If a student is having trouble in an area, additional help is available. If class work is not completed, the student risks termination and may only be readmitted upon the director’s approval. Once class work and test have been completed, a certification test is available for each program. There will be three (3) 50 question exams which includes final exam, and two (2) quizzes. All exams/quizzes are multiple choice, fill in the blanks, true and false and essay style questions. The grading scale is as follows: 100-90% A 89-90% B 79-75% C Below 75 % D Program Failure: If a student fails the Nurse Aide program may have a chance to re-enroll in another session. Before reenrollment students must meet with instructor and school administrator to evaluate student academic history, attendance, lab and clinical practicum. Students are not guaranteed re-enrollment. If students are accepted, students may enter the program as a new student and pay required tuition/fees. Graduation Requirements: Students must have an overall score of 75% or higher to successfully complete the Nurse Aide Training Program and complete a total of 75 clock hours for theory, lab and clinical with 100% accuracy for lab practice and clinical practicum. Students must have a thorough understanding of the class and complete all course work before a certificate is given. At the end of the program students will receive a certificate 8 Volume 2 10/27/13 of completion from Advanced Care Training and will be eligible to sit for the Michigan Nurse Aide Certification Exam. Rules of Student Conduct and Conditions for Probation or Dismissal: Students are required to conduct themselves in an adult manner at all times during class, lab and clinical. Students are expected to exhibit characteristics of professionalism, punctuality, and reliability at all times. Training will take place on the premises of New Providence Baptist Church, and students are expected to follow rules and regulations of the church as well as rules and policies put in place by Advanced Care Training. Smoking/Drugs/Alcohol: Smoking is prohibited on school grounds or on the premises of New Providence Baptist Church. If a student appears to be intoxicated or impaired due to the use of drugs or alcohol, immediate termination from the program will take place. Cell Phones: Cell phones are to be kept on vibrate, and should be answered during break times. We at Advanced Care understand if a student must answer an emergency call during class time. No cell phones will be allowed during clinical. Visitors: Students are not allowed to bring guest or children to the school for any reason during class, lab or clinical sessions. Weapons/Violence/Profanity: Students who bring weapons of any kind on school/church property, who intentional hurt, harm or threaten a student / staff or use profanity towards students/faculty will immediately be terminated from the program and dismissed from school property. Sexual Harassment: Our goal at Advanced Care Training is to maintain a safe and comfortable environment for our students. Any student who makes sexual advances, request sexual favors, engage in inappropriate touching or use force of coercion will immediately be terminated from the program. Cheating: Cheating is prohibited. Students who copy examinations and distribute to other students for improvement of grades will be dismissed from the program. Dress Code/Uniform: Students are required to dress professionally at all times. No extreme hair color, make up or nail colors are accepted. Females must keep nails cut low with neutral color. Hair is to be neat at all times. Tattoos must be covered. Body piercings of any kind must be removed prior to class. Students are encouraged to not wear jewelry, or heavy perfumes. Clothes are to be neat, ironed/pressed, clean and of perfect fit. Spandex, tank tops, hats, or open toe shoes are not to be worn. Students are required to wear Navy Blue scrub top and pant with white lab coat if needed. White scrub shoes are also required. Students must wear identification badges at all times. Badges are issued during enrollment process. 9 Volume 2 10/27/13 Grievance Policy: If a student disagrees with a decision or action taken by an instructor may submit their disagreement by email to: ljennings@advancedcaretrain.com within two days of occurrence. A meeting may be scheduled if necessary. If the grievant fails to attend the meeting then the original action or decision will remain in effect. Students must first follow chain and command and speak with instructor to resolve any issues. If there is no resolution students may speak with school administrator. The final decision will be given to the grievant in writing with in seven (7) business days. Students who wish to file a complaint with the State of Michigan may do so at www.michiganps.net. Description of Facilities: Advanced Care Training is located inside of New Providence Baptist Church of Detroit. Lectures will be held in an appropriate class room setting with adequate lighting and temperature. Laboratory practice will take place inside of the Medical Ministry room located inside of New Providence Baptist Church. The Medical Ministry room is equipped with hospital beds, cabinets stocked with personal care supplies, durable medical equipment, full body manikins, and sinks. Students are asked to park in the parking lot close to Plymouth Road for class. Students must sign in at the beginning and end of class. Break times will be 12-12:45pm daily with two (2) 15 minute breaks. Rest rooms for student use are also located on the second floor with classrooms. Equipment Furnished: The student must bring pens, pencil and a notebook to class. Advanced Care will provide equipment for training purposes such as, training stethoscopes, blood pressure cuffs, durable medical equipment (beds, wheelchairs, walkers, commodes,) manikins, and personal care supplies. Clinical Practicum: Clinical times are from 7am-3pm, Monday- Wednesday. Students are responsible for transportation to and from clinical site. Students will not be paid during clinical rotation. Uniforms must be worn during clinical rotation. Special Needs Students: Advanced Care Training for Healthcare Professionals is committed to providing equal access to all persons with disabilities.
