Name Change Passport Application (PC8 Form)
Your application must be lodged in person, by Appointment Only.
Please call the Passport Section on 022 799 9100 to make an appointment.
Incomplete applications will be rejected and you will be required to attend a second interview to lodge.
Include with your application:
1. Completed PC8 application form
2. Proof of citizenship, either A or B:
A. Original full Australian birth certificate (Australian birth extracts & commemorative certificates
are not accepted)
B. Original Australian citizenship certificate and Original foreign birth certificate (an International
version otherwise an official certified English translation must be provided)
3. Proof of name change (see below)
4. Proof of ID documents – one document from each category, A, B and C:
A. Original valid photo ID (foreign passport, driver’s license or residence permit)
B. Original valid other ID (health insurance card, credit card, bank card or Social Security card)
C. Proof of residential address – Original utility/phone/medical bill or bank statement
5. Proof of the mother’s maiden name (family name at birth) – Original birth or marriage certificate or the
Swiss family book
6. Expiring/expired passport (this will be returned with the new passport)
7. Two compliant passport photos: (https://www.passports.gov.au/Web/Requirements/Photos.aspx). One
photo must be correctly endorsed and signed by the Guarantor. Please DO NOT attach the photos with
paperclips, tape or glue. Leave them loose and we will attach them.
8. Proof of payment (if applicable, see fee waiver notice and payment details below)
9. Self-addressed return envelope and stamps (CHF7) for registered return mail or you can collect in person.
Please note that passports can only be mailed within Switzerland.
Important - Please note that we do not accept the Swiss Family Book (certificat de famille /
Familienausweis / Certificato di famiglia) as proof of name change.
Processing Time:
Processing will only commence once we have received the COMPLETE application form and supporting
documents, including correct photos and payment. You should normally allow up to three weeks to receive a new
passport. Incomplete applications will be rejected.
Proof of Name Change - Marriage:

Born and Married in Australia - Original marriage certificate issued by the Register of Births Deaths and
Marriages (for RBDM offices in Australia see below).

Born Overseas and Married in Australia - Original marriage certificate issued by the Register of Births
Deaths and Marriages (for RBDM offices in Australia see below).

Born in Australia and Married Overseas - Original name change certificate or revised birth certificate
issued by the Register of Births Deaths and Marriages (for RBDM offices in Australia see below) and the
Original foreign marriage certificate. The foreign marriage certificate will need to be the International
version otherwise an official certified English translation is required.

Born and Married Overseas - Original foreign marriage certificate stamped with an Apostille(*).The
marriage certificate will need to be the International version otherwise an official certified English
translation is required.

Proof of Name Change - Divorce:

Born and Married in Australia - Original name change certificate issued by the Register of Births Deaths
and Marriages (for RBDM offices in Australia see below).

Born in Australia and Married Overseas - Original name change certificate or revised birth certificate
issued by the Register of Births Deaths and Marriages (for RBDM offices in Australia see below) and the
Original divorce papers. If the documents are in a foreign language then an official certified English
translation of all pages is required.

Born and Married Overseas - Original divorce papers stamped with an Apostille(*). If the documents are
in a foreign language then an official certified English translation of all pages is required.

Please check the following before lodging your application:





Was the form completed using a BLACK biro pen?
Did you supply only ORIGINAL documents?
Did you check your PHOTOS for size and quality?
Did the GUARANTOR fill out all the required sections?
Did the GUARANTOR correctly endorse and sign the back of one PHOTO?

Fee Waiver Due to Name Change:
Should you wish to replace your current passport due to a name change (this applies to persons who change their
name due to entering or leaving a registered or de facto relationship or a result of transitioning to another gender),
you will be eligible for a fee waiver when applying for a replacement passport. Your current passport must be valid
for a further two years to qualify and the expiry date of the new passport issued will remain the same as your
current passport.
Payment:
We accept payment in Swiss Francs only. Payment can be made in cash only or by bank transfer providing the
proof of payment at the time of your interview. We do not accept any card payments. Please note that passport fees
are subject to change on the 1st of each month. Please consult our website for the most up-to-date fees
(http://www.australia.ch/gene/cons_pass_fees.html). No refunds will be given for applications that are withdrawn.
