Box 107, Serena, IL 60549 To: From: Date: All Parents of District Students Marty Felesena, Superintendent May 21, 2015 OPENING OF SCHOOL INFORMATION Registration Procedures All students in the district are requested to register and pay fees at the Serena High School cafeteria and not at your child’s school. The dates for registration are Tuesday, August 4th, from 8:00 a.m. to 3:00 p.m. and Wednesday, August 5th, from 12:00 p.m. to 7:00 p.m. Checks should be made payable to CUSD2. One check may cover all fees for all students you may have attending any school in the District. Most forms needed for registration may be found on the CUSD No. 2 website at www.unit2.net under Parents, Student Registration Information. All students must have a new enrollment form completed every year. In order to reduce the amount of time you will spend at registration, please complete all forms and bring them with you to registration. We will have laptops set up at registration so that you may pay for registration, lunch, and other fees through the Illinois E-Pay System. The system allows you to use credit cards, bank cards, and/or electronic checks for payment of all school-related charges. This consists of three pieces of information proving residency. Such items as a driver’s license, utility bill, or rental agreement will be sufficient. You will need to sign a records release form so that we may have access to your child’s records from his/her previous school. In addition, we will need some form of proof of your child’s identity such as a birth certificate as well as medical and immunization records. Also, please keep in mind that your child may be placed in any school in the District based on class sizes and/or bus routes. Mandatory Fees Fees for students include a book rental fee, an activity fee, and a technology fee. Fees for the 20152016 school year are: Grades K-4 = $65.00, Grades 5-8 = $70.00, Grades 9-12 = $75.00. The fees for students attending Indian Valley Vocational Center is $60.00, but the Center will have additional fees. The activity fee provides student plan books and admission to all school sponsored sporting events and plays free with a valid student identification card. Admission to tournaments sponsored by or in relation to the IESA, IHSA, Little 10 and Fox Valley Conferences are not included in the activity fees. NEW…The technology fee provides a personal technology device to each student. NEW…There will be a $250.00 family cap on the above-mentioned fees. 1 | Page Optional Fees The athletic participation fee for grades 4-8 is $40.00 for the first sport and $25.00 for each additional sport. The athletic participation fee for grades 9-12 is $50.00 for the first sport and $35.00 for each additional sport. There is a music program participation fee for high school students of $15.00 per program. The band instrument rental fee is $25.00 per semester. The high school driver education fee is $175.00. The high school parking fee will be $40.00 for student drivers. They will receive their driving identification tag with that fee. Breakfast/Lunch Program A breakfast program and a lunch program are available for all students. The cost for K-12 Student Breakfast will be $1.50. The prices for lunch will be $2.65 for grades K-8; $2.85 for grades 9-12; $2.75 for ala carte main course for grades 9-12; and $3.65 for adults per day. Extra milk is available for $.35 per half-pint. Enrolling in the Illinois E-Pay system allows us to use your child’s student ID card as a debit card to pay for their school lunch or milk. There is a bar code on the student ID card that will be scanned when they go through the lunch line and the cost of lunch or milk will automatically be debited from the account you establish for your child. You may pay for breakfast and lunch daily, by the week, or by the month. Students must have money in their account in order to eat each day. If students do not have enough money to cover their account, they will be able to get lunch one day. Any day thereafter, they will only be able to get a peanut butter sandwich, apple, and milk until money is placed in their account to cover for previous and future purchases. P.E. Locks/Uniforms Freshman students will have a fee of $5.00 for a combination padlock for a physical education locker. Grade school and high school PE uniform order forms will be available at registration this year. Parents/guardians can order and pay for uniforms at registration and uniforms will be handed out to students when school begins. There will be sample uniforms available to look at during registration to help with choosing sizes. Prices will be $10 for shorts and $6 for shirts. Payments (Illinois E-Pay) Unit District 2 has initiated the capability to take electronic payments for all school charges and fees through the Illinois E-Pay system provided by the Illinois State Treasurer’s Office. By doing this, we make it more convenient for you and we expand our capability to accept fees and other school charges. This system allows you to use credit cards, bank cards, and/or electronic checks for payment of all school-related charges, including food service. We can use your child’s student ID card as a debit card by scanning the bar code on the back of the card and automatically debiting your account. To do this, you will need to go to the E-Pay section on the Community Unit School District No. 2 website (www.unit2.net), select the fees and payments that you need to make, and enter information similar to any online purchase (name, credit card number, address verification, etc.). Once the transaction is complete, both you and the District will be provided with a printable receipt. 2 | Page These transactions will be entered daily into our student management system, TeacherEase. In order to utilize this system, you must establish an account with TeacherEase. To access TeacherEase, please go to www.teacherease.com. If you don’t have an account with TeacherEase or do not remember your username and password, please contact your child’s school. Physical Examinations All students entering school for the first time (kindergartners or out-of-state), all sixth graders, and all freshmen must have a physical examination before school begins. Extracurricular Participants All extracurricular participants must have a physical examination before they may participate in any athletic practice or event. The sixth grade and/or freshman physical will suffice for the athletic physical requirement for that year. Immunizations/Screenings The following immunizations are a part of the physical examination and must be completed, or if a series of inoculations is required, a physician must certify in writing, that the series has begun. Measles Children enrolling in the sixth grade must provide acceptable documentation of two doses of live measles virus vaccine, with the first dose administered not earlier than 12 months of age and the second dose no less than one month later. If a student has been vaccinated at 12 months, but less than 15 months, that student must receive a second dose in order to be admitted to school for the first time. A student who has been vaccinated at 15 months or later will be considered in compliance until the time of entry into the fifth grade. Mumps Children entering school at any grade level (kindergarten through 12) shall show proof of having received two doses of live mumps virus vaccine, the first dose on or after the first birthday and the second dose no less than four weeks (28 days) after the first dose, or other proof of immunity. For students attending school programs where grade levels (kindergarten through 12) are not assigned, including special education programs, proof of having received two doses of live mumps virus vaccine shall be submitted prior to the school years in which the child reaches the ages of 5, 11, and 15. Rubella Children entering school at any grade level (kindergarten through 12) shall show proof of having received two doses of live rubella virus vaccine, the first dose on or after the first birthday and the second dose no less than four weeks (28 days) after the first dose, or other proof of immunity. For students attending school programs where grade levels (kindergarten through 12) are not assigned, including special education programs, proof of two doses of live rubella virus vaccine shall be submitted prior to the school years in which the child reaches the ages of 5, 11, and 15. Hepatitis B Children entering the sixth grade shall show proof of having received three doses of hepatitis B vaccine, or other proof of immunity. The first two doses shall have been received no less than four weeks (28 days) apart. The interval between the second and third doses shall be at least two months. The interval between the first and third doses shall be at least four months. Proof of prior or current infection, if verified by laboratory evidence, may be substituted for proof of vaccination. Varicella Children entering kindergarten, sixth grade, or ninth grade for the first time shall show proof of having received two doses of varicella vaccine, the first dose on or after the first birthday and the second dose no less than four weeks (28 days) after the first dose, or proof of prior varicella disease, or laboratory evidence of varicella immunity. For students attending school programs where grade levels (kindergarten through 12) are not assigned, proof of having received at least two doses of varicella vaccine or other proof of immunity shall be submitted prior to the school year in which the child reaches the ages of 5, 11, and 15. Invasive 3 | Page Children under two years of age entering a child care facility or school program below the kindergarten Pneumococcal level shall show proof of immunization that complies with the pneumococcal vaccination. Children 24 Disease to 59 months of age who have not received the primary series of pneumococcal conjugate vaccine, according to the recommended vaccination schedule shall show proof of receiving one dose of pneumococcal vaccine. Any child who has reached his or her fifth birthday shall not be required to provide proof of immunization with pneumococcal conjugate vaccine. The requirements of this section also apply to children who transfer into Illinois child care facilities, school programs, and schools from other states, regardless of the age or grade level at which the child transfers. Pertussis To address the increase in Pertussis (Whooping Cough) cases among older students, a booster vaccination (called Tdap) is recommended for all students in grades six through twelve. Beginning in 2014, students entering sixth and ninth grades will be required to provide proof of Tdap vaccination along with the school physical forms that are also required for these grades. Lead Screening A lead screening is required for students ages 6 or below who are entering kindergarten in all state licensed or approved schools and programs. Diabetes A diabetes risk assessment is a required procedure for all physical examinations. Eye Exam An eye examination is also a required procedure for all physical examinations. Dental Examinations All children in Kindergarten, 2nd grade, and 6th grade of any public, private or parochial school shall provide proof of a dental health examination. In addition, each of the children shall present proof on the appropriate form of having been examined by a dentist before May 15 of the school year. If a child in grade 2 or grade 6 fails to present proof by May 15 then the school may hold the child’s report card until one of the following occurs: 1. The child presents proof of a completed dental examination; 2. The child presents proof that a dental examination will take place within 60 days after May 15; or 3. The parents obtain a waiver from the Department of Public Health or provide objection to the dental examination on religious grounds. The Illinois Department of Public Health shall establish a waiver for children who show an undue burden or a lack of access to a dentist. Each public, private, and parochial school must give notice of this dental examination requirement to the parents and guardians of students at least 60 days before May 15 of each school year. Bus Routes Information on bus routes and pick-up times for the 2015-16 school year will be available during registration (August 4th and 5th). Times may vary somewhat during the first few days of school. School Starting Times Serena Grade School and Sheridan Grade School begins classes at 8:10 a.m. Harding Grade School begins classes at 8:05 a.m. Serena High School has an attendance period beginning at 8:10 a.m. Normal dismissal time will be 3:01 p.m. for Serena Grade School and Sheridan Grade School, and Serena High School. Harding Grade School will dismiss daily at 2:56 p.m. Kindergarten 4 | Page The District operates an all-day kindergarten program. The kindergarten will be on the same schedule as other grades. It is important to note for the 2015-16 school year, kindergarten students may be assigned to any of the attendance centers in order to balance out enrollment. Serena/Sheridan Grade Schools, Serena High School will attend from 8:10 a.m.-3:01 p.m. Harding Grade School from 8:05 a.m.-2:56 p.m. Please note that the first day of attendance, Tuesday, August 18th, will be a full day of school.
