Seeking VCS representatives to input into the redesign the local health & social care services across children and adults services. (Devolution) Health & Social Care Forum are managing the process for recruiting voluntary & community sector representatives in the Governance of the devolution pilot. The Devolution pilot has an ambitious programme of developing an integrated effective and financially sustainable system that covers the whole range of wellbeing, from public health initiatives for school children, timely and appropriate access to GPs and community pharmacists, and top quality hospital treatment, to excellent mental health services and supporting people to remain independent in their community for as long as possible. This is a partnership of ten of the borough’s key health and social care providers and commissioners. Hackney Council, City and Hackney Clinical Commissioning Group, Homerton University Hospital Foundation Trust, East London Foundation Trust, City and Hackney GP Confederation, City and Hackney Urgent Healthcare Social Enterprise, City and Hackney Pharmaceutical Committee, Healthwatch Voluntary & Community Sector via the Health and Social Care Forum We are seeking representatives, who will be accountable to the Voluntary & Community Sector via the networks managed by Hackney CVS, and who can consult with, and take forward the views of the wider sector for the below workstreams: If you are interested in being the representative on any of these workstreams please email Jackie@hcvs.org.uk by the 8th January and I will send you the role description, application form a fuller draft terms of reference for each workstream. Representatives will be selected following panel interviews with members of the HSCF Steering Group. Workstreams are: Best Practice and Outcomes – Co-chairs Deborah Colvin (GP Confederation) and Martin Kuper (HUHFT) Draft Purpose: To ensure sound clinical evidence underpins any service re-design or new model of care, maximising the opportunities for improving care and building on the work of our existing clinical boards, CCG work groups and other local fora. The workstream will also help clarify professional decision-making and accountabilities in the new model, and evaluate options for improving communications across the system. System and organisational design – proposed chair Tracey Fletcher (HUHFT) Draft Purpose: To outline the proposal with greater definition about opportunities for innovation and delivering local benefits. To produce the outline business case. This workstream will involve Hackney patients/services users, as well as clinicians and practitioners. Communications and public engagement: Co-chairs: Catherine Macadam (PPI CCG Board Chair) and Jon Williams (Healthwatch) Draft Purpose: To ensure early service user and public engagement and involvement as proposals develop, and to receive timely feedback. The workstream will develop and implement a detailed plan for stakeholder communications, and maintain an active log of meetings and input received at each stage. Finance, financial modelling and commissioning - Chair Paul Haigh (CCG) Draft Purpose: Finance, financial modelling and commissioning Chair Paul Haigh (Chief Officer, CCG) Purpose: To understand the resources available to Hackney from devolution and to provide financial analysis and modelling for the outline business case. This workstream will also design the most cost effective way to deliver residual commissioning responsibilities and working with other systems. Maintain an oversight of robustness of all business cases, financial models etc. Estates (buildings) Co-chairs Philippa Lowe (CCG) and Ian Williams (LBH) Draft Purpose: To assess current estates and consider areas to pilot during 2016-17, making a recommendation to the Executive Group In addition we are seeking VCS representation on the IT Enablers Group Purpose The key purpose of the Board is to oversee delivery of IT enablers to ensure the implementation of an integrated system of prevention, wellbeing, care and support that result in better health and social care outcomes. As part of these responsibilities, the ITHSCR Board will have strategic responsibility for the identification, development and implementation of IT solutions and will be responsible for programme management of their delivery. This board will feed into the Devolution process later in the year. City & Hackney Advisory Board, this is a subsidiary of Hackney Council for Voluntary Services, it has been set up to enable Hackney’s voluntary and community sector to respond to the emerging preference on the part of local commissioners to have fewer or single contracts with larger providers. The Advisory Group is made up of local voluntary and community organisations, including local infrastructure bodies, and makesdecisions about eligibility for membership of City and Hackney Together as well as the composition of bidding consortia.
