Division of Integrated & Community Services The District Nursing service is undergoing changes over the next 12 months - this information is valid to 6th April 2015. NORTH MANCHESTER DISTRICT NURSING REFERRAL CRITERIA & SERVICE RESPONSE District Nurses provide skilled Nursing care in the home and residential care homes 24 hours a day/365 days a year service for patients and families. We also provide clinic based treatment room services 6 days a week. Our work includes: Rehabilitation following a debilitating illness, Wound care, Advice on promoting and maintaining a healthy lifestyle, Care after inpatient & day care surgery, continence and bowel care and providing support and treatment for people suffering with long term conditions. We have a key role in the care of people with life threatening illnesses and this includes providing support and advice to patients, families and carers in the last year of life, with intensive support during the last few weeks of life, especially in relation to pain and symptom relief enabling patients to die in their preferred place of care. Aims of the service: To provide professional nursing care, advice and support to patients who have acute, chronic and terminal conditions, by agreeing an individualised plan of care with the patient. To improve quality of life, maximise independence and, wherever possible, prevent inappropriate admission to and facilitate safe and timely discharge from hospital. Home Visiting Service The home visiting service is available to people aged 16 and over who are referred with a specific nursing need and who are in one of the following groups of people who: are unable to leave their home environment through a physical and/or psychological illness require a home visit from their General Practitioner are temporarily unfit to travel following an operation require end of life care are neutropenic and undergoing chemotherapy, radiotherapy or whose health condition would lead to travel or attendance at a clinic being detrimental to their treatment or recovery have been identified by the District Nurse as needing case management for a health need in the professional opinion of the District Nurse, treatment or care would be appropriate to be carried out in the person’s own home Division of Integrated & Community Services Treatment Room Service Patients who fall outside the home visiting criteria will be offered an appointment at a local treatment room. When to refer Supporting patients/carers to meet their individual or palliative care needs including liaising with other appropriate agencies. Wound management/Post-operative care advice. Preventing further complications associated with an existing illness/disability. Providing acute intervention to patients with chronic degenerative conditions. Teaching and educating patients/carers in reaching their optimum potential, and where appropriate facilitating referral to other agencies. To undertake health needs assessments for patients on their caseload in conjunction with Social Care to ensure patient needs are met. Screening/public health activities in line with local and national initiatives. Assessment co-ordination and management of complex, chronic and acute nursing needs Holistic wound assessment and management Acute and chronic disease management Palliative care for those who are terminally ill and care of the dying Technical nursing interventions, e.g. syringe drivers, central lines, intravenous drugs, etc Bereavement support Promotion of continence and management of incontinence Assessment of nursing equipment Health promotion for individuals, carers and communities Nurse prescribing Nursing procedures: ear care, injections, immunisations and vaccinations Home based Phlebotomy The District Nursing service currently only undertake home based phlebotomy for patients already on their caseload. Hours of Service The service operates 24 hours per day over a 7 day period. Making a referral Referrals are accepted by fax, and email. The District Nursing Referral form is attached. Division of Integrated & Community Services Treatment Room Treatment room appointments are available Monday – Saturday at a number of community locations. For non-house bound patients able to attend a treatment room, appointments can be made at any centre by contacting 0161-274-1640 (Ancoats Health Centre). The referral does not need to be sent to the District Nurses. Treatment room sessions and access times will be changing as from 7th April 2015. They cover the whole of the North Manchester area for all the GP’s listed above. The team have an answer machine which is remotely accessed throughout the night. Service response Following a referral the district nursing team will prioritise the visit using the following criteria:Urgent – Immediate access within 4 working hours: this could include end of life patients who want to be cared for at home; visits that can prevent an unnecessary attendance at A&E or unnecessary hospital admission and can be safely managed at home Non–urgent – Access within 24–48 hours: patients in need of curative care e.g. acute and chronic wound management, post–operative patients, medications. Routine – Access at a convenient time: for example bereavement visits, flu immunisations, blood tests and continence assessments. Patients or carers will be contacted within 24 hours of receipt of referral and an appointment will be made for the first visit.
