ACNP Job Description
Position Summary
The Adult Gerontology Acute Care Nurse Practitioner (AG-ACNP) will use knowledge
and expertise of adult-gerontology population to appropriately manage care of acute and
chronically ill patients with various cardiac disease processes, improve patient outcomes, and
promote overall well-being through continued collaboration with members of an interdisciplinary
healthcare team.
Qualifications

Must hold a Master’s degree with a nursing focus from an accredited nurse practitioner
program.

Must be a board certified Acute Care Nurse Practitioner.

Required to hold current state licensure from the Ohio Board of Nursing

Must hold current Certificate of Authority privileges granted through Ohio Board of Nursing.

Required to be within good standing with the State of Ohio Board of Nursing.

Must be proficient with computers including knowledge and experience with Microsoft
word, Microsoft excel, Microsoft power point, and electronic medical records, preferably
EPIC.

Three years critical care experience as a bedside registered nurse within a hospital setting.

Must have completed basic 12 lead EKG class provided by hospital or by nurse practitioner
program within past three years.

Additional cardiology experience within the hospital, outpatient cardiology setting, or
graduate course work with cardiology focus is preferred, but not required.

Required to hold and maintain current ACLS and BLS certifications.

Will need to show proof of current malpractice insurance and must maintain insurance for
duration of employment with Premier Health.

Must complete standard care agreement within one month of hire in collaboration with
Miami Valley Cardiologists and in accordance with rules/regulations of Premier Health.
Reporting Structure/Evaluation

The AG-ACNP will report directly to the collaborating physician(s) with whom the AGACNP has contracted the standard care agreement.

The AG-ACNP will participate in bi-annual review and evaluation given details outlined in
standard care agreement with aforementioned collaborating physicians, making revisions to
agreement as needed.

The AG-ACNP will complete a yearly self-evaluation that will reviewed by the collaborating
physicians to whom the AG-ACNP reports.
Roles and Responsibilities
Clinician

Performs a comprehensive or qualified problem focused history and physical exam on
patients with both acute and chronic cardiac illnesses.

Synthesizes available data to prioritize appropriate interventions for acute and chronically
ill patients.

Orders appropriate diagnostic testing for patients with acute and chronic cardiac illnesses.

Prescribes medications, interventions, and durable medical equipment for responsible
patient population.

Effectively navigates electronic medical record to obtain patient information.

Keeps detailed, timely, and up to date records for all patient encounters via electronic
medical record.

Successfully communicates plan of care in daily cardiology notes in an acute care setting.

Discusses and promotes individualized plan of care through the use of therapeutic
communication with patients and their families.

Maintains continuity of care with patients with routine follow-up in outpatient setting as
needed.
Collaborator

Successfully conveys patient needs (serves as patient advocate) with appropriate
physicians and other members of the interdisciplinary healthcare team.

Serves as a cardiology consultant for other physician groups in the acute care and
outpatient setting.

Works in concord with collaborating cardiologists in acute care and outpatient setting.

Works together with nurses, other physician groups, and patients in the acute care setting
to develop an individualized, appropriate, and progressive plan of care for each patient.

Serves as a beneficial member of an interdisciplinary healthcare team to improve
outcomes of critically ill patients and their families.
Educator/Researcher

Routinely reviews and utilizes the most up to date evidence based practice protocols for
management of cardiac illnesses in the acute care setting.

Provides education to patients and their families admitted for acute cardiac related
illnesses.

Counsels patients and families on health maintenance and illness prevention methods
based on age appropriate educational and cultural needs.

Seeks out continued professional learning opportunities through yearly national
conferences.

Serves as a preceptor to aspiring acute care nurse practitioner students following one year
of experience.

Participates in publications and research opportunities in an effort to maintain current
knowledge of acute and chronic cardiac illnesses to improve the outcome of patients.

Works on a regular basis and complete required hours needed to fulfill the job.

Maintains composure at all times in the work place, will work efficiently, and will work
well under pressure when various job constraints arise.
Professional Leadership

Exhibits a positive attitude and continued willingness to learn or develop new skills.

Displays flexibility with physician, patients, and staff needs within the practice.

Actively practices the “patient’s first” philosophy in direct conjunction with Miami
Valley Cardiologists and Premier Health mission statement.

Effectively uses the chain of command to deal with patient complaints or problems
between office personnel.

Attends monthly or quarterly staff/practice meetings and serves as committee member or
chair for the group when needed.

Maintains competency of local, state, and national healthcare initiative and legislation in
order to practice within the full scope of their practice and provide continual

improvement in patient access to healthcare and improvement in outcome in acutely and
chronically ill patients.

Maintains knowledge of scope of practice and confidently practices within that scope.

Serves as a member of peer review board for performance evaluations of peers within the
practice.
