DUTIES/RESPONSIBILITIES:

 The incumbent, a professional social worker, in this position functions as the Case

Manager for VA Sierra Nevada Health Care System Mental Health Intensive Case

Management Program (MHICM). The primary responsibilities of this position is to provide intensive case management services, continuous quality improvement activities, evaluation/consultation, psychosocial assessments, patient groups, individual therapy, placement services, and referrals to various community agencies and hospital services to MHICM patients. MHICM patients are seriously mentally ill veterans with a high percentage of co morbid substance issues who have been identified as high utilizers of psychiatric inpatient hospital resources. MHICM patients have previously not been able to achieve adequate community adjustment with traditional psychiatric care.

The patient caseload is complex based on the severity of the mental illness, co morbidities, and resistance to standard treatments. Treatment interventions are innovative and highly individualized. Patients are referred to the MHICM program from the outpatient psychiatry clinics and the inpatient psychiatric unit.

 This position is located within the Mental Health Intensive Case Management (MHICM)

Program in Mental Health Clinic. The incumbent must possess the clinical skills to provide treatment for addictions and mental health disorders.

BASIC REQUIREMENTS:

U.S. Citizenship: Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy

English Language Proficiency: Individuals appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and

7407(d)

Education: Possess a Master's Degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)

Licensure: Applicants must be currently licensed or certified by a state to independently practice social work at the advanced practice level

Additional Requirements for GS-12: Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice. Senior social workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty (see paragraph

3a(c))

Experience refers to paid and unpaid experience, including volunteer work done through National

Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
