1. QUESTIONNAIRE ADMINISTRATION
Provide a brief description of how the institution ensures that all AdvancED surveys were administered
with fidelity to administration procedures. This should include the following information:
•
Document how you attempted to meet the minimum response rates for each survey group
(parents – 20%, staff – 60%, and students 40%).
•
Describe accommodations you provided to participants (i.e. paper surveys for those
without internet access, surveys in alternate languages, etc.).

This will help you support and further address Evaluative Criterion 2. It will also help as you complete
