Document1Document11 Community Clinical Objectives Compliance A student reflection Narida Morle New York City College of Technology Document1Document12 Abstract This paper is a basic evaluation of how I met the clinical objectives required by the course; which is Community health nursing. I provide the clinical objectives as a platform for readers to compare my response to, which will help decide if the clinical objectives had been successfully fulfilled. This community setting that I was assigned to fulfill these nine clinical objectives is a group of facilities that are assisted facility homes that are geared toward the mental health population. The facilities also serve the homeless and previous foster children who consenting adults in need of housing. Community access introduces innovative ways to approach this population and deal with their life issues; I had the honor to be a part of the process and I prove that with this paper. Document1Document13 Community clinical objective compliance A student reflection The goals that I wanted to accomplish within this course are ways to promote health within the mental health population, improvement of communication between client and nurse as well as disease prevention along with the maintenance of health. With these various goals being set in place the collaboration with my group had been a through communication with a common idea in mind. We wanted to share ways in which we could tackle all of these factors but in a creative way that enhanced the clients compliance with medications, special diets and treatments as well as educating this population. We had to think of way to do this because it is not easy to encourage individuals but we faced a special challenge because the mental health population have an increased difficulty communicating with others. These goals had been reached through using fun ways to interact and educate the population as well as using laymen terminology. The most important thing had been allowing them to freely express themselves and trying to understand what they were trying to express by rewording and asking if that was correct. The benefits that the student learning project has brought to the community is awareness. Making the community aware of their limitations and what they should be doing to improve them. It also made us as student and nurses more conscious of our community and how important communication as well as collaboration is to improve the health of any community. By working with other student nurses we collaboratively created ways to manage and prevent diseases that plague communities. This project brings light to the issues that many individuals face in regards to drug use and chronic Document1Document14 diseases which are all preventable health problems. These goals are further supported with the corresponding compliance with the nine clinical objectives. Objective 1: Demonstrates individual professionalism through personal behaviors and appearance. During my community access site rotations I demonstrated professionalism with the use of my appearance, confidentially compliance, and active participant in postconferences as well as assignment completion. I can support my claim of maintaining residents confidentially, with a situation I had experienced with a resident, which was of a personal discussion about a health related issue; in which the resident preferred it be kept private, as it was not a harm to himself or others so I fulfilled his wishes. In general all information shared throughout the community access site had been shared with necessary persons exclusively. Maintaining the confidentially of the residents, improved the trust within the relationship. I had upheld a friendly and mature manner during group sessions, casual encounters, individual discussion with residents and staff. The moments that a residents developed an issue, I sought after the appropriate persons to advocate for the residents to improve his/her well-being. All while, I continued to have composure and a professional presences. Objective 2: Employ analytical reasoning and critical thinking skills when providing care to individuals and families in the community setting. Within the community access sites, I had the opportunity to interview the residents and obtain various information about their health; I discussed ways to improve their health states and prevent the development of other diseases or worsening of Document1Document15 current diseases. Eliciting information from staff about the resident’s diagnosis and more importantly, how diseases are being maintained with the use of prescribed medications and treatments. Engaging in interviewing and thorough communication with staff had been a way I provided care although in an in-direct manner. I educated the residents about health related topics such as nutrition; more specifically how proper nutrition can improve health outcomes and improve diseased states. In addition to nutrition education, I educated on factors that can be a hindrance or in contrast improve high blood pressure. The residents had been very receptive to the information and verbalized and understanding of the information given Objective 3: Effectively communicate with diverse groups and disciplines using a variety of strategies regarding the health needs of individuals and families in the community setting. The mental health population had been a major group within the community setting I served. With the mental health population, there can be great difficulties with communication due to the nature of the mental health diseases. I had to formulate unique methods to communicate with the residents effectively, which involved using a fifth-grade reading level when creating learning materials. As well as using a technique that required the residents to engage themselves within the discussions that were about health and social issues. Engagement had been a way to get into the minds of the residents and persuade them to have a differing outlook on certain things. On another note, during post-conferences, my communication with peers and the instructor had been well spoken and professional. Objective 4: Establish environment conducive to learning and use a plan for learners based on evidence-based practice. Document1Document16 During teaching sessions, I made sure the environment was free of distractions, calm and open. All the residents that attended the session were respectful and in a mood conducive to learning. The residents had been optimistic and engaged in the group discussions; they would ask questions and share thoughts about certain topics. My teaching plan had been properly planned and done in a presentation format which promoted an environment that everyone was in attention to the knowledge that was shared with them. Many of the residents expressed understanding the education sessions, but during my evaluations I was convinced they had learned, because they explained what they understood and actually demonstrated it. For example, a resident chose brown sugar to sweeten her coffee as opposed to white sugar and artificial sweeteners such as “sweet and low”. The resident had learned from a session of the good and bad of artificial sweeteners and white sugar versus the good and bad of brown sugar. Objective 5: Utilize informational technology when managing individual and families in the community. The community access site of East Broadway and Stanton mainly used handwritten charting to document information about the residents health related information. The site used technology to document the resident’s information, although you needed to be a staff member to have access to electronic records; I understood the importance and function of the electronic system. Objective 6. Demonstrate a commitment to professional development Document1Document17 I am a highly committed nurse and lifelong learner of innovative ways to treat the individuals of the mental health population. Through the use of nyc.gov, the world health organization and my pyramid as resources to educate this group. Objective 7. Incorporate professional nursing standards and accountability into practice Within the community setting, I was in compliance with the nursing process of assessment, planning, outcome setting, intervening and evaluation per the American Nurses Association of standards. During the care I provided ethics and social standards had been upheld. The residents respect and dignity had not been tampered with as well as the allegiance to do no harm. The community access mission had been fulfilled within their identified philosophical theory. Objective 8. Collaborate with clients, significant support persons and members of the The residents that had been need in intervening, ranging from health related to socially related; I ensured the residents the resident received the necessary help. Objective 9. Recognize the impact of economic, political, social and demographic forces that affect the delivery of health care services I identified the gaps in the care system being a multitude of factors; the mental health disease in itself is a gap to proper health care because the residents may not have proper judgment and impulsive in the decisions that are made. Due to the mental health populations lack of rationality, there is an interference with the coordination of care. As a group member of the provision of the student learning project, I identified various means to close the gaps as well as becoming a change agent. I have successfully met most of the clinical objectives within the assigned community setting and it has had an impact on my growth as a new nurse.
