Age & Marking Instructions for the Vet
Breeder’s Annual Declaration
ALL FOALS MUST BE MICROCHIPPED and can also be BRANDED
All outline diagram forms and DNA forms must be filled in by a qualified vet.
There are three outline diagrams which need to completed :

Outline diagram on the National Equine Database Form (white card)
Outline diagram on reverse of Initial Registration Form (blue card)
The yellow copy of last year’s covering certificate. (This is a declaration of birth which goes to the
APSL in Portugal.) Covering certificates should be given to mare owners by stallion owner, filled in
with covering details. Covering certificates are available from the Breed Society.
Please return all forms and hair sample to me together with a cheque for £110 made payable to
Lusitano Breed Society for initial registration with LBSGB, APSL & DNA typing. If a passport is also
required please add a further £40 (LBSGB members) £50 (non-members)

Signed Breeder’s Annual Declaration
The foal must now be microchipped by a qualified vet to enter the book of births in Portugal.
You may also send in white copies of any coverings done this year. Information on coverings must be
submitted by August 1st of the covering year.
