Case Management Team Leader Job Description Overall Purpose Provide leadership to staff members responsible for resourcing CPO residents including but not limited to those responsible for the Neighborhood Networks, Referrals Upon Request, At-Risk Resident and Service Coordination programs. Manage collaborations and processes that effectively link CPO residents with necessary resources, opportunities and processes that increase their housing stability and ability to move forward with their lives. Coordinate the intake process for CPO residents to enter the CPO Employment Program and provide case management to active participants. Reporting Relationships Report to the Supportive Services Manager VISTA volunteers, OSU Interns Key Accountabilities 1. Manage collaborations and processes that effectively link CPO residents with necessary resources, opportunities and processes that increase their housing stability and ability to move forward with their lives. a. Provide case management services to CPO residents enrolled in the CPO Employment Program. 3. Maintain resident records according to standard procedures. 4. Actively participate in inter-departmental collaborative processes. a. Attend and participate in inter-departmental case staffing meetings b. Develop and maintain thorough understanding of property management practices relevant to maintaining housing (eviction process, fair housing, lease agreements, HUD and tax credit policies, etc.) 5. Work with Neighborhood Coordinator to build and manage a collaboration of service organizations, neighborhood groups, social service agencies as a “Justice League” in relationship with residents of CPO neighborhoods. a. Identify and invite community based programs, agencies, faith-based groups, whose mission it is to increase safety and resources for populations that match the population living with CPO, to be members in the “Justice League”. b. Ask participants at Neighborhood Network gatherings the Miracle Question in order to identify additional “Justice League” members. c. Host and facilitate quarterly “Justice League” meetings with a focus on giving and getting feedback to ensure a “win – win” experience for all participants. (e.g. New members are oriented to the Neighborhood Network processes, members share relevant information, and barriers to resourcing residents are problem-solved) 6. Work with Neighborhood Coordinator to develop and oversee an outreach plan with Engagement Specialists to effectively link CPO residents with other neighborhood programs and services. 7. Work with Neighborhood Coordinator to coordinate and implement monthly Neighborhood Network gatherings in neighborhoods where CPO residents reside. a. Manage gathering logistics (childcare, refreshments, etc.) b. Facilitate the gatherings utilizing solution-focused strategies. c. Ensure each resident in attendance receives relevant follow-up utilizing Engagement Specialists and “Justice League” members. 8. Help develop and manage evaluation plan to measure outcomes and to inform continuous improvements of Neighborhood Networks. a. Maintain all documentation according to established procedures. b. Develop and distribute satisfaction surveys to participants of gatherings and “Justice League” members on a semi-annual basis. c. Commit to continuous improvement processes. d. Provide data and information necessary to complete funder and agency reports.
