Ty Gwyn Philosophy
The philosophy of the multidisciplinary team at Ty Gwyn Care Home is
a Holistic approach to patient care, which involves physical and social
rehabilitations.
We strive:
 To attain the highest standard of performance possible for our
residents individual needs
 To treat residents with courtesy and respect
 To treat residents equally regardless of racial or ethnic origin,
religious beliefs, sec, age, disability or sexuality
 To respect the residents right to privacy and confidentiality
 To maximise residents independence within their abilities
 To provide an education resource for residents and carers
Work Objectives
 To provide high quality, professional, patient centred care based
on holistic approach, working within a multidisciplinary
framework
 To effectively deliver individualised nursing care using a named
nurse and key worker approach
 To protect and act in the best interests of the residents at all times
 To advocate for those residents unable to represent themselves
 To provide a safe environment for residents and staff
 To involve relatives/carers in the delivery of care
Learning Outcomes
This placement will assist you to develop the knowledge, skills and
attitude required to enable you to care for the older person using a
holistic approach.
Under supervision, you will deliver nursing care to residents including:
 Personal Hygiene – assisting to wash, dress, mouth care etc.
 Pressure Area Care – pressure ulcer prevention. Appropriate use
of pressure relieving equipment, turn charts and risk assessments.
 Diet & Fluid Intake – maintain/improve nutritional intake,
including the management of residents requiring enteral feeding,
special diets and nutritional risk assessment.
 Elimination – continence care, including catheter and bowel care.
 Communication – development of communication skills when
dealing with deaf/blind/dysphasic residents.
 Psychological Care – developing a rapport with residents to
promote trust and confidence between nurse and resident.
 Wound Care – wound assessment, selection of appropriate
dressings and aseptic technique.
 Infection Control – applying prevention and infection control
interventions as per policy.
 Co-Ordinating Care – liaising with other disciplines and relatives.
Learning Opportunities
To assist you in achieving your learning outcomes, there are various learning opportunities that may be available to students at
Ty Gwyn. These need to be discussed and agreed with your mentor/s. You will be able to:
 Attend CHC (Continuing Health Care) review meetings with UHB (University Health Board) and Social Workers.
 Shadow the weekly GP ward round (Dr. Butterworth)
 Spend time with other members of the multidisciplinary team i.e. Physiotherapist, Activities Co-Ordinator and Chaplain.
 Attend domiciliary assessments with the Manager and her Deputy.
 Spend time at Morel Court MHA Residential Home and Live at Home scheme located in Penarth.
