REQUEST FOR PROPOSALS
Be part of the 2014 HCAW Annual Conference, “Bold New World of Healthcare at Home”
scheduled for April 22 - 24, 2014 at the DoubleTree Suites Hotel at Southcenter. Many
healthcare system changes have been in process resulting in the dawning of a new day for our
industry. The landscape is changing and the pieces are starting to fall in place; however, we
recognize that we still have a long way to go. What will the final picture will look like? What
strategies must we have for success in the new world? Please help us answer these questions
by presenting at our 2014 Annual Conference.
The Annual Conference has been delivering exceptional education, unique learning, and quality
networking opportunities for several decades – and we plan to continue to raise the bar at the
2014 program. In order to do so, we are seeking your creative participation. Suggested topics
include, but are not limited to: managing change, LEAN or cost savings measures, partnership
development, payment reform, quality performance, grant opportunities/grant writing, current
state of state health reform, Legislative issues, new trends, innovations in delivery of in home
services, technological advances, survey issues, palliative care, emergency preparedness.
Please complete the following form and submit by September 30, 2013 to be considered as a
presenter at HCAW’s 2014 Annual Conference.
LEAD FACULTY CONTACT INFORMATION
Please remember that HCAW offers complimentary registration for the Lead faculty. This
registration entitles faculty to attend all education programs, general sessions, and the Vendor
Fair for the day of their presentation. HCAW does not provide honorarium or expense
reimbursement for faculty.

ABOUT YOUR PRESENTATION
All presentations are 60 minutes long and must include an interactive portion. HCAW will not be
printing any materials, but will instead provide electronic versions of your presentations to
attendees.
Intended Audience
Please indicate your intended audience for this RFP only.
Home Health: agencies providing skilled health care and supportive services,
utilizing nurses, therapists (physical therapists, occupational therapists, speechlanguage pathologists, respiratory therapists), medical social workers, home health
aides, and often other services, such as nutrition counseling and pharmacy.
Home Care: agencies providing non-professional supportive services, such as
home care aides, chore, respite and companion care.
Hospice: agencies providing medical, social, psychological and spiritual care for
terminally ill patients and their families. In addition to the full array of skilled
professionals employed by home health agencies, hospices also utilize chaplains
and bereavement counselors, and sometimes other services such as massage
therapists.
All In-Home Services
1. Provide a program description that enables attendees to select the presentation that will best
meet their needs. Limit to 200 characters (approximately 40 words).
2. List your presentation title and 3 major learning objectives.
3. Describe the intended audience in terms of their years of experience, job titles, skill levels, or
other relevant criteria.
4. Describe how your presentation supports the professional development of those in the inhome services field.
5. Provide a brief outline of the content to be presented.
6. Provide a copy of your presentation outline and include a rough timing sequence. In this
outline, describe the training modalities (lecture, group discussion, individual cognitive activities,
experiential activities, etc.) you use to actively engage the audience.
7. Describe any takeaway materials that you provide to the audience.

Upon receipt, your submission will be acknowledged and forwarded to the review selection
committee. Each member of the review team independently evaluates all proposals and ranks
them according to selection criteria established by the Conference Committee. This ranking
determines the proposals selected by the review team.
If your proposal is accepted, HCAW will request a Speaker Commitment Form along with
providing a schedule of deliverables.
