NHS Tayside JOB DESCRIPTION – LOCUM CONSULTANT BREAST SURGEON Introduction The area of NHS Tayside coincides with that of the Tayside Region. It encompasses the former counties of Angus, Perth & Kinross together with the cities of Dundee and Perth, and the Burghs of Arbroath, Forfar, Brechin and Montrose. NHS Tayside currently provides a service to Tayside and North Fife - a population of up to one million. Dundee is Scotland’s fourth largest city and lies 20 miles east of Perth on the north bank of the River Tay. It houses two large Universities (Abertay and Dundee) and is the focal point for the ongoing development of a thriving biomedical technology industry. Perth is the regional centre for a predominantly rural county of Perthshire and Kinross. Several international companies have their headquarters in Perth. The population is increasing and the area offers a very favourable life style with its location and facilities. Both cities, together with the historic City of St Andrews in Fife, provide a mixture of traditional and modern living and the surrounding area provides an extensive choice of housing and schooling. Most leisure activities are available close by with particularly good facilities for hill-walking, climbing, ski-ing and golf. NHS Tayside All acute services in the region are provided by NHS Tayside. NHS Tayside has merged the acute clinical services provided at Ninewells Hospital, Perth Royal Infirmary, Stracathro Hospital and Arbroath Infirmary. The Acute Balance of Care Project governs optimum service delivery in Tayside with emphasis on sustaining PRI as a major hospital site in Tayside. Breast Surgery is managed within the Clinical Group of Specialist Services and both Ninewells Hospital and Perth Royal Infirmary provide a symptomatic and screening breast service. Close clinical, research and training links exist between the two sites engendering a “two sites one service” philosophy. This new post will be based in the Breast Unit at Ninewells Hospital & Medical School, Dundee, and sessions at Perth Royal Infirmary will be available. The Hospitals Ninewells Hospital and Medical School is a major general teaching hospital and provides a comprehensive range of services with specialist surgical units. Ninewells Hospital and Medical School is one of five designated Regional Cancer Centres in Scotland and provides on a single site all Surgical, Radiotherapy, and Medical Oncology services. The internationally renowned Cuschieri Surgical Skills Centre is housed in Ninewells Hospital and Medical School. NHS Tayside and the University of Dundee have a number of joint professorial appointments in Surgical Oncology, Clinical Oncology and Cancer Medicine. Perth Royal Infirmary general surgical unit provides acute services for Breast, Colorectal and surgical Gastroenterology. A 36 bedded Surgical Unit is located in the new development at Perth Royal Infirmary. Perth Royal Infirmary is recognised as a Cancer Unit for Tayside. There are close links between clinical practice and laboratory and clinical research in the University providing the opportunity to develop research interests.The major clinical Oncology Services complement the international standing of laboratory Biomedical Research under the Principal of the University of Dundee, Professor Peter Downes. The Ninewells site has the new Clinical Research Centre which opened in early 2008 for clinically based research including phase 1,2 and 3 breast cancer trials and an interventional suite suitable for 3T MRI directed breast surgery. Linked to this is the Translational Medical Research Centre, the hub for Scotland’s translational medical research initiative. The provision of cancer services across Tayside is coordinated by the Tayside Cancer Network. The breast sub-group is represented on the Tayside Cancer Network and has produced local protocols and guidelines for the management of Breast cancers that are in line with national guidelines. The Tayside Cancer Network is part of the regional North of Scotland Cancer Network (NOSCAN) ensuring uniformity and quality of service for all breast cancer patients in the region. The Surgical breast service within Ninewells Hospital is is provided by the breast surgical team Mr DC Brown Mr J Macaskill and Ms MM Reis and three plastic and reconstructive surgeons with an interest in breast reconstruction, Mr A Munnoch, Ms F Hogg and Mr M Tohill All aspects of breast cancer surgery are provided including the full range of breast reconstruction and the specialist techniques of sentinel node biopsy and intraoperative radiotherapy. 7 specialist breast care nurses within the unit provide cancer care and support across Tayside. There is a weekly breast Multidisciplinary meeting with contributions from surgeons, oncologists, radiologists, pathologists, nurses and clinical trials personnel. The preoperative assessment, operative options and postoperative management of all patients with cancers of the breast is optimised in these meetings according to national and local guidelines with a significant number being entered into neo-adjuvant and adjuvant national and international trials. All aspects of breast cancer management are provided in the unit. Over 450 new breast cancers are treated annually through the symptomatic and screening service. The symptomatic clinic is run as a one stop service with immediate mammographic and cytological reporting. The East of Scotland Breast Screening Service, Director Mr DC Brown, is coordinated through Ninewells and provides screening and assessment for women aged 50-70 across Angus, Tayside and North East Fife. Approximately 18 000 women are screened annually with an attendance rate of 79%. Surgery for impalpable disease is performed both in Ninewells and Perth Royal Infirmary. There is a well developed Family History Breast Service within Ninewells run jointly between the breast team and geneticists who provide annual review of high risk women with immediate mammography and genetic counselling. A breast MRI service has recently been introduced and provides input to this clinic. Ninewells has an established Plastic Surgery Unit with 3 consultants, Mr A Munnoch, Ms F Hogg and Mr M Tohill providing input to the breast service including an immediate and delayed reconstructive service for patients undergoing mastectomy. The Breast Unit transferred to the Specialist Services Clinical Group two years ago to form a joint Breast/Plastics Unit in Ward 27. The Oncoplastic Service continues to develop with regard to joint operating sessions and possible joint clinics. Locum post The Locum Consultant Surgeon appointed will have a commitment to providing surgical services for patients with breast disease in Tayside on a 10 session/week basis. It is expected that the new appointment will contribute to out-patient clinics and theatre sessions within the Tayside network at Ninewells Hospital and Perth Royal Infirmary– see job plan. It is expected that the successful candidate will have a central role in the running of a joint oncoplastic service with the plastic surgeons within the Oncoplastic Unit. Experience in breast reconstructive surgery is therefore desirable. In addition experience and expertise in surgery for impalpable disease is desirable. Audit The Trust recognises the need for quality assurance, the provision of healthcare and the appointee will be required to play a leading role in this area in the Group. The Group is revising the Clinical Governance structures and the appointee will be required to play a leading role in this area. Appraisal NHS Tayside has implemented an annual appraisal system for all consultant staff. The successful candidate will be required to actively participate in this scheme. Conditions of Service The terms and conditions of employment will be subject to the Trust Terms and Conditions of Service for Medical Staff as agreed locally with the BMA Local Negotiating Committee.
