UPLSAA DISTINGUISHED ALUMNUS AWARD 2014
Information and Guidelines
To give acknowledgment to the UP School of Library and Information Studies (UP SLIS)
alumni who have given pride and recognition to the Library and Information Science
profession, the UP Library Science Alumni Association (UPLSAA) will once again hold the
nomination for the UPLSAA Distinguished Alumnus Award.
THE NOMINATION PROCESS
The UPLSAA Distinguished Alumnus Award will be conferred to one (1) alumnus who has
demonstrated exceptional achievements and outstanding contributions in the field of
library and information science and thus has given distinct honor to the University. This
year, recognition will be given particularly for achievements and contributions that impact
the community in which the individual is or has been active, either at the local, national or
international level.
Nominations for these awards may be made by any UPSLIS alumnus.
Evaluation of nominees and selection of awardee will be made by the selected UPLSAA
Panel of Judges and affirmed by the UPLSAA Board of Directors.

GENERAL CRITERIA FOR THE AWARD
Evaluation of nominees for the Distinguished Alumnus Award will be based on the
following general criteria:
SERVICE ORIENTATION – the nominee’s work has clearly benefited other people, groups,
and society at large, and not merely private or personal interests; 20%
LEADERSHIP – the nominee has demonstrated effective and inspiring personal leadership
in accomplishing the work for which s/he is being awarded; i.e., achievements can be
attributed more to the person than to the official position/job/title s/he has held; 20%
IMPACT – the nominee’s work has had a substantial impact on the people, geographic area,
or sector covered by the award, both qualitatively (how deep, lasting or transformative)
and quantitatively (how many people or groups benefited within the area or sector); 30%
INTEGRITY – there is no serious concern about, or challenge to, the nominee’s personal
integrity. 10%
CONTRIBUTION TO THE PROFESSION - the nominee's continuous contribution to the
profession. 20%
Distinguished Alumni Awardee is required to personally receive his/her award during
the 64th UPLSAA Alumni Homecoming on 10 May 2014 at the Ang Bahay ng Alumni in
the UP Diliman Campus.

INSTRUCTIONS AND IMPORTANT INFORMATION FOR NOMINATORS:
1. Nominations for the Distinguished Alumnus Award shall be made by accomplishing
and submitting a copy of the corresponding nomination form.
2. All documents in support of the statement of achievements, such as copies of
diplomas, certificates, citations, news articles, photographs, etc., may also be
submitted. If possible, at least three copies of the nominee’s recent passport
photograph (2”x2”) should be submitted.
3. Nominations must be delivered or mailed to: UPLSAA Committee on Alumnus
Award c/o UP School of Library and Information Science, Gonzalez Hall, UP Diliman,
Quezon City. Electronic mail submission is also allowed, provided that the
nomination form is scanned and sent to the e-mail address: uplsaa@yahoo.com.
4. Nominations should be received no later than 21 March 2014 and/or postmarked
no later than the above date. Only complete and properly accomplished forms shall
be accepted.
5. The UPLSAA Committee on Alumnus Award will deliberate on the nominations and
may request additional information or documents from the nominators. The
Committee’s decision shall be final, and will be submitted to the UPLSAA Board for
affirmation.
6. The UPLSAA will immediately notify the awardee of her/his selection for the
award. In the event that the awardee declines to accept the award and/or fails to
attend the awarding ceremonies, the UPLSAA reserves the right to withdraw the
award.

7. The UPLSAA reserves the right to keep two (2) copies of the nomination papers and
to dispose of the rest not claimed after two (2) weeks from the award day.
8. The awardee must attend the awarding ceremonies during the General Alumni
Homecoming and Reunion on Saturday, 10 May 2014 to personally receive his/her
award.
